Zusammenfassung
Kritisch kranke Patienten entwickeln mit einer variablen Inzidenz Funktionsstörungen des Gehirns mit negativen Auswirkungen auf die Prognose der Grunderkrankung im Sinne einer erhöhten Mortalität, einer verlängerten stationären Verweildauer, höheren Behandlungskosten sowie prolongierten kognitiven Defiziten. Das peri- und postoperative Delir kann somit als Versagen der Organfunktion „Zerebrum“ angesehen werden; außerdem kann ein Delir frühzeitig auf andere systemische Störungen (z. B. Sepsis) hinweisen. Insbesondere bei traumatologischen Patienten liegt häufig eine Alkoholanamnese vor, sodass mit der Ausbildung eines Alkoholentzugssyndroms gerechnet werden muss. Wichtig sind die Kenntnis der Risikofaktoren und die konsequente Diagnostik und Früherkennung. Sowohl medikamentöse als auch nichtmedikamentöse Maßnahmen können eingesetzt werden, um den Verlauf und die Schwere des Delirs positiv zu beeinflussen.
Abstract
Delirium occurs in a large variety of patients in the intensive care unit or after severe injury. The incidence is reported at up to 80% in these patients. Delirium is associated with an increased mortality, increased length of hospital stay and higher costs for treatment. Delirium can be regarded as a “cerebral failure” indicating systemic diseases such as sepsis and may lead to prolonged cognitive disorders. In trauma patients, alcohol withdrawal syndrome may be a serious complication in the perioperative period. Besides screening for risk factors, critical care patients should be routinely assessed for delirium and treated immediately by pharmacological and non-pharmacological measures to improve clinical outcome.
