Mittelfuß- und Zehenfrakturen

Zusammenfassung

Der Fuß hat als peripherstes Glied des Bewegungsapparats entscheidenden Anteil an der Geh- und Stehfähigkeit des Menschen. Verletzungen im Bereich der Metatarsalia und Zehenglieder verlangen aufgrund ihrer Bedeutung für das statische Gerüst des Fußes obligatorisch eine detaillierte Diagnostik und Therapie. Abrissfrakturen der Peronaeus-brevis-Sehne an der Metatarsale-I-Basis, Jones-Frakturen und dislozierte Schaftfrakturen der Metatarsalia werden häufig einer operativen Therapie zugeführt. Dabei kommen Osteosynthesen mit perkutan eingebrachten Spickdrähten sowie Miniplättchen oder Kleinfragmentschrauben zur Anwendung. In allen übrigen Fällen sollte eine konservative Therapie mit möglichst kurzer Ruhigstellung und rascher Wiedererlangung der Vollbelastung erfolgen, um Kontrakturen und nicht zuletzt auch einer Dystrophie vorzubeugen.

Abstract

The foot is the furthest distal part of the locomotor system and as such plays a crucial part in the human being’s ability to stand and walk. When the metatarsal bones and the phalanges of the toes are injured, detailed diagnostic investigations and therapy are therefore mandatory because of the importance of the potential effect of such injuries on the static platform of the foot. Fractures of the bony insertion of the peroneus brevis tendon at the base of the 1st metatarsal bone, Jones fractures and dislocated metatarsal shaft fractures are often treated surgically with percutaneous K-wire osteosynthesis, miniplates or small-fragment screws. In all other cases conservative therapy with the shortest possible period of immobilization and early full weight-bearing is indicated, to prevent contractures and not least also dystrophy.
