Zusammenfassung
In der heutigen Therapie von Frakturen spielt die Weichteilbehandlung eine herausragende Rolle, insbesondere auch – aufgrund der anatomischen Gegebenheiten – bei der Pilon-tibiale-Fraktur.Osteoartikulär gesehen ist für ein gutes langfristiges Resultat eine strikt anatomische Rekonstruktion der Gelenkfläche zu fordern. Bisherige operative Behandlungstechniken sind mit einer relativ hohen Rate an Weichteilkomplikationen verbunden.Die hier vorgeschlagene Technik beinhaltet nicht nur die Verwendung eines neuen Implantats, sondern auch eine angepasste Weichteilbehandlung und -schonung.Bei der Implantation wird außerdem auf maximale Versatilität geachtet.Zur Reposition und Fixation der gebrochenen Gelenkfläche können verschiedene Schraubendurchmesser konventioneller Art sowie Schrauben mit rigidem Plattenverbund (locking screws) zum Einsatz kommen.Der epiphysäre Teil der Platte ist somit als Verbund von Plattenlöchern konzipiert und zur achsengerechten Fixation am Tibiaschaft mit einem konventionellen, einfachen geraden Plattenteil verbunden.Die Platte kann nach Bedarf gebogen und geschnitten werden und somit dem gewählten Zugang und der Plattenlage, die antero-lateral, antero-medial, medial oder posterior sein kann, angepasst werden.
Abstract
There is an emphasis on treatment of the soft tissues in modern fracture care.This is especially true in the case of tibial plafond (pilon) fractures, because of the local anatomy. As far as the bone and joint are concerned, satisfactory long-term results are dependent on a strictly anatomical reconstruction of the articular surface. In the past, operative treatment of these fractures has been linked to a high rate of complications affecting the soft tissues.The technique presented in this paper is based on a new implant, which is designed in such a way that the implantation can be carried out with a minimum of soft tissue impairment. In addition, during the operation emphasis is placed on maximal versatility of the plate at the level of the joint.For anatomical reduction and fixation of the fractured joint surface, different diameters of both conventional screws and locking screws can be used.The epiphyseal part of the plate is thus conceived as an assembly of mechanically linked screw holes (as opposed to a plate with screw holes). A conventional straight part is designed for alignment of the reconstructed epiphysis to the tibial shaft.The plate is designed in such a way that it can be bent and also cut to shorten it, which means it can be tailored to the location, which can be antero-lateral, antero-medial, medial or posterior, and to the specific approach selected.
