Zusammenfassung
Hintergrund: Das Frey-Syndrom ist eine häufige Komplikation nach operativen Eingriffen an der Ohrspeicheldrüse. Ziel dieser Untersuchung war es, die Prävalenz des gustatorischen Schwitzens am eigenen Patientengut zu ermitteln und den Therapiebedarf zu quantifizieren.
Patienten und Methode: Untersucht wurden 69 Patienten mit Zustand nach lateraler Parotidektomie bei Adenomen der Glandula parotis. 43 Patienten (62%) gaben an, unter dem Frey-Syndroms zu leiden, und 19 Patienten (28%) fühlten sich durch die Symptome in ihrer Lebensqualität beeinträchtigt.
Ergebnisse: Bei den Patienten, die bisher kein gustatorisches Schwitzen bemerkt hatten, konnte in 85% der Fälle eine subklinische Manifestation nachgewiesen werden. Insgesamt 33 von 43 klinisch vom Frey-Syndrom betroffenen Patienten wünschten eine Behandlung. Bei 8 Patienten wurde ein Heilversuch mit intrakutaner Injektion von Botulinumtoxin A durchgeführt. Innerhalb 1 Woche kam es zum vollständigen Sistieren des gustatorischen Schwitzens.
Schlussfolgerungen: Bei Patienten mit ausgeprägtem gustatorischen Schwitzen stellt die Behandlung mit Botulinumtoxin A eine sichere und effektive Therapie dar.
Abstract
Background: Gustatory sweating is a common complication of parotid surgery.¶Patients and methods: In order to evaluate the incidence of Frey’s syndrome following superficial parotidectomy, 69 patients who underwent surgery due to adenoma were studied. Forty-three patients (62%) suffered from gustatory sweating following superficial parotidectomy, and 33 of them requested treatment. Nineteen patients felt that their quality of life had been decreased by the symptoms.
Results: Minor’s starch iodine test proved that 85% of the patients who did not notice Frey’s syndrome after surgery actually had a subclinical manifestation. Eight patients were successfully treated with intracutaneous injections of botulinum toxin A. Within 1 week gustatory sweating disappeared.
Conclusion: Frey’s syndrome is present in almost all patients following superficial parotidectomy and there is a strong need for treatment. Intracutaneous injection of botulinum toxin A is an effective treatment in severe cases of the syndrome.
