Zusammenfassung
Nicht nur die Therapie der peripheren arteriellen Verschlusserkrankung (PAVK) wird komplizierter je mehr Möglichkeiten bestehen. Auch die Wahl der Therapie ist für den verantwortlichen Arzt schwerer geworden, da er/sie sich in einem Spannungsfeld zwischen Patientenorientierung und -verantwortung auf der einen und der wirtschaftlichen Verantwortung für den Betrieb (Praxis und/oder Krankenhaus) auf der anderen Seite befindet. Am Beispiel der PAVK im stabilen Stadium der Claudicatio wird dieses Spannungsfeld aufgezeigt. Dabei begünstigt die Vergütung in der Regel aufwendigere Prozeduren oder Operationen, während konservative Therapieansätze schlecht entlohnt werden, auch wenn diese den aufwendigeren Prozeduren oder Operationen in Bezug auf den Extremitätenerhalt und das Gesamtüberleben nicht unterlegen sind.
Abstract
The treatment of peripheral arterial occlusive disease (PAOD) has become more complicated because an increasing number of options are available. The choice of treatment has also become more difficult not only because doctors have to realize their responsibility towards the patients but also the economic responsibility towards the institution (private practice or hospital) they are working for. This conflict of priorities is described exemplified by PAOD in the stable stage with intermittent claudication. The German reimbursement system as a rule favors the remuneration of more complex interventional or operative procedures, while the remuneration for conservative treatment is lower, even though its effect is not inferior to the more complicated procedures or surgery with respect to limb preservation and long-term survival.
