Zusammenfassung
Die im Juni 2012 veröffentlichte Leitlinie zur Lokaltherapie chronischer Wunden mit den Risiken periphere arterielle Verschlusskrankheit (PAVK), Diabetes mellitus (DM) und chronisch venöse Insuffizienz (CVI) hat mangels Verfügbarkeit methodisch guter Studien mit hochwertiger Evidenz viele Fragen in Hinblick auf die konkrete Anwendung der zur Verfügung stehenden Wundbehandlungsprodukte offen gelassen. Dennoch gibt es keinen Zweifel, dass die moderne hydroaktive Wundtherapie nicht nur aus medizinischen, sondern auch aus ökonomischen Gründen den allgemein akzeptierten Standard in der Wundbehandlung darstellt. In der Praxis sollte sich die Auswahl der geeigneten Lokaltherapie im Rahmen einer phasenadaptierten Wundversorgung vornehmlich an der Exsudataufnahmekapazität des Verbandes unter Berücksichtigung der Patientenpräferenzen orientieren. Im Rahmen dieser Publikation werden auf der Grundlage einer innerhalb der Wundkommission der DGG abgestimmten internen Expertenmeinung praxisorientierte Handlungsempfehlungen zu den Kapiteln Wundreinigung, Wundspülung, Débridement, periodische aktive und passive Wundreinigung, Dekontamination, Antiseptika und wirkstofffreie Wundauflagen der S3-Leitlinie dargestellt.
Abstract
The guidelines published in June 2012 on local treatment of chronic wounds with the risks of peripheral arterial disease (PAD), diabetes mellitus (DM) and chronic venous insufficiency (CVI) have, in the absence of available methodologically good studies with high-quality evidence, left many open questions with respect to the concrete use of the available wound care products. However, there is no doubt that modern hydroactive wound care is the widely accepted standard in the treatment of chronic wounds not only for medical but also for economic reasons. In practice the choice of the appropriate local therapy in a phase-adapted wound treatment should primarily be orientated in to the exsudate uptake capacity of the dressing taking into account patient preferences. As part of this publication practical recommendations of the S3 guidelines are presented in the sections on wound cleansing, wound irrigation, debridement, periodic active and passive wound cleaning, decontamination, antiseptics and drug-free wound dressings.
