Zusammenfassung
Weil Beckenarterienstenosen mit relativ guter Prognose durch PTA (mit Stent) therapierbar sind, ist eine exakte Graduierung hämodynamisch relevanter Stenosen zur adäquaten Therapieentscheidung wichtig. In Fallbeispielen werden die farbduplexsonographische Stenosegraduierung sowie die methodenspezifischen Schwächen radiologischer und sonographischer Bildgebung erläutert. Der Quotient aus intra- und prästenotischer systolischer Spitzengeschwindigkeit ist ein geeignetes Kriterium zur Stenosegraduierung. Eine zeiteffiziente Untersuchung wird durch Analyse des Dopplerfrequenzspektrums in der Leiste ermöglicht, seine Triphasigkeit als einziges Kriterium zum Stenoseausschluss ist jedoch nicht ausreichend treffsicher (Kollateralisierung). Zusätzlich müssen die Akzelerationszeit und die systolische Spitzengeschwindigkeit im Seitenvergleich beurteilt werden.
Abstract
Because treatment of iliac artery stenoses with PTA (with stenting) has relative good prognosis, precise grading of hemodynamically relative stenoses is important to make adequate treatment decisions. Using exemplary cases, color duplex ultrasound grading of the stenoses and the methodological weaknesses of x-ray and ultrasound imaging are discussed. The ratio of the intra- and prestenotic peak flow velocity is suitable for stenosis grading. A time-efficient examination is made possible by Doppler frequency spectrum analysis of the groin; however, its triphasic form as the sole criterion for exclusion of stenosis is not sufficiently accurate (collateralization). In addition, the comparison of acceleration time and peak systolic velocity of both sides is necessary for decision-making.
