Zusammenfassung

Die präoperative Evaluation und das perioperative Management der koronaren Herzerkrankung bei Patienten mit vaskulären Eingriffen stellt eine große Herausforderung mit bedeutenden Auswirkungen für das operative Ergebnis dar. Myokardinfarkt und kardialer Tod sind die Hauptursachen der perioperativen Mortalität von Gefäßpatienten. Einen wesentlichen Bestandteil der Risikoreduktion stellt die perioperative medikamentöse Therapie dieser Patienten dar. Anhand der aktuellen Literatur sowie europäischen und amerikanischen Leitlinien wird die Bedeutung von β-Blockern, Statinen und Thrombozytenaggregationshemmern für die Senkung der perioperativen Morbidität beleuchtet.

Abstract

Preoperative evaluation and perioperative management of ischemic cardiac disease are a great challenge in patients undergoing major vascular surgery. They have an important impact on perioperative morbidity and mortality in patients with atherosclerotic disease needing surgery. Perioperative medication is most important in reducing these risks. This review summarizes the current literature and European and American guidelines on the perioperative use of beta-blockers, statins and platelet aggregation inhibitors and their impact on reducing perioperative morbidity.
