Zusammenfassung
Trotz optimaler Untersuchungsergebnisse der Ultraschalldiagnostik bleiben für die aszendierende Pressphlebographie wichtige Indikationen bestehen. Durch die Anwendung des Überlaufeffekts gelingt auch routinemäßig eine gute Darstellung der Wadenmuskelvenen.
Zur Abklärung des akuten Wadenschmerzes stehen die Sonographie, die Computer- und die NMR-Tomographie an erster Stelle. In phlebologischer Hinsicht geht es dabei um die Erkennung einer Thrombose der Wadenmuskelvenen. Bei 10% der Fälle ist eine Ausschlussdiagnose nicht mit letzter Sicherheit möglich, sodass hier die Indikation zur Phlebographie gegeben ist. Es lassen sich auch kleinste Thromben in den Klappentaschen als Monokel- und Brillenzeichen nachweisen.
Zur Diagnostik des postthrombotischen Syndroms der Wadenmuskelvenen ist die Phlebographie allen anderen Verfahren überlegen. Indikationen ergeben sich besonders in der Unfallchirurgie und bei der Begutachtung. Als neues typisches Röntgensymptom werden die Wirrnetze beschrieben.
Degenerative Veränderungen der Wadenmuskelvenen zeichnen sich durch einen typischen phlebographischen Aspekt aus. Inwieweit sie als Ursache von uncharakteristischen Beschwerden in Betracht kommen, ist individuell zu entscheiden. Mit der aszendierenden Pressphlebographie werden sie eindeutig diagnostiziert.
Abstract
Despite optimal results with ultrasonographic diagnostics, there are still important indications for ascending press phlebography. Through the application of the overflow effect, a good representation of the calf muscle veins can be obtained routinely.
Sonography and computed and NMR tomography are the first-line methods for finding causes of acute calf pain. Phlebologically, it is a question of recognizing thrombosis of the calf muscle veins. In 10% of cases, definite differential diagnosis is not possible, and this is the indication for phlebography. With this method, even the smallest thromboses in the valve cusps can be discerned, appearing in the shape of monocles or eye-glasses.
For the diagnosis of a post-thrombotic syndrome of the calf muscles, phlebography is superior to all other methods. It is called for particularly in emergency operations and insurance appraisal. The new and typical roentgenological symptom of a tangle of veins is described.
Degenerative changes in the calf muscle veins are shown by a typical phlebographic sign, but it remains to be determined how much they may seen as the cause of uncharacteristic maladies. These can be clearly diagnosed with ascending pressphlebography.
