Zusammenfassung
Der vorliegende Artikel gibt einen Überblick über die Erkrankungs‑, Sterbe- und Überlebensraten sowie die Stadienverteilung von Tumoren im Kopf-Hals-Bereich in Deutschland. Die Ergebnisse basieren auf Daten der bevölkerungsbezogenen Krebsregister der Bundesländer, die im Zentrum für Krebsregisterdaten auf Bundesebene ausgewertet werden. Etwa 4560 Frauen und 12.660 Männer erkrankten im Jahr 2014 an einem Kopf-Hals-Tumor. Tumoren an Mundhöhle und Lippen sowie Oropharynxkarzinome wurden dabei am häufigsten diagnostiziert. Bei den Männern sind die Erkrankungsraten seit Ende der 1990er-Jahre, außer für Oropharynxkarzinome, rückläufig, wohingegen sie bei Frauen insgesamt, aber v. a. für Neubildungen der Mundhöhle und des Oropharynx angestiegen sind. Geschlechtsunterschiede, aber auch Unterschiede zwischen den Lokalisationen, zeigen sich ebenfalls für die Stadienverteilung und die Überlebensraten. Während bei den Rachentumoren rund drei Viertel der Tumoren bereits ein Stadium IV (A–C) aufweisen, liegt dieser Anteil bei den übrigen Lokalisationen unter 50 %. Die höchsten relativen Fünf-Jahres-Überlebensraten zeigen sich für Tumoren der großen Speicheldrüsen (75 %) und des Nasopharynx (69 %) bei Frauen sowie für Tumoren des Kehlkopfs (62 %) und der Nase/Nasennebenhöhle (57 %) bei Männern. Für fast alle Lokalisationen sind die Stadienverteilung und Überlebensraten bei Frauen günstiger als bei Männern. Die für beide Geschlechter im Vergleich zu anderen Lokalisationen etwas ungünstigeren zeitlichen Trends beim Oropharynxkarzinom sind auch in anderen Ländern zu beobachten und deuten wahrscheinlich auf eine zunehmende ätiologische Bedeutung von HPV-Infektionen hin.
Abstract
This article provides an overview of the incidence, mortality and survival rates as well as the stage distribution at diagnosis for cancers of the head and neck in Germany. The results are based on population-based cancer registry data of the federal states, which are analyzed at the federal level by the German Center for Cancer Registry Data. In 2014 approximately 4560 women and 12,660 men were diagnosed with cancer of the head and neck region. Tumors of the oral cavity and lips as well as tumors of the oropharynx were most frequently diagnosed. Among men the incidence rates have been declining since the late 1990s, except for oropharyngeal cancer, whereas the incidence rates among women have increased overall but especially for neoplasms of the oral cavity and oropharynx. Differences can be found between sex and localization and are also present for stage distribution and survival rates. Whereas three quarters of pharyngeal tumors are diagnosed at stage IV (A–C), for other tumors of the head and neck this proportion is below 50%. The highest relative 5‑year survival rates are observed for tumors of the large salivary glands (75%) and nasopharynx (69%) among women as well as for tumors of the larynx (62%) and nasal cavity/paranasal sinuses (57%) among men. For nearly all localizations, the stage distribution and the survival rates are more favorable among women compared to men. Compared to the other localizations, in both sexes oropharyngeal cancers have a more unfavorable incidence trend, which can also be seen in other countries. This trend probably indicates an increasing etiological relevance of human papillomavirus (HPV) infections for these tumors.
