Zusammenfassung
Hintergrund
Dem Patientenwillen, der in Patientenverfügungen für den Fall der fehlenden Selbstbestimmungsfähigkeit zum Ausdruck gebracht wurde, ist bei der medizinischen Versorgung Rechnung zu tragen.
Ziel
In diesem Beitrag werden das Konzept des Advance Care Planning (ACP) sowie Auswirkungen auf die Patientenversorgung unter Berücksichtigung von Entscheidungssituationen in der Onkologie referiert und unter klinisch-ethischen Gesichtspunkten diskutiert.

Selektive Literaturrecherche in PubMed von deutsch- und englischsprachigen Publikationen zu Advance Care Planning.
Ergebnisse und Schlussfolgerung
Patientenverfügungen, die nicht mit Ärzten oder Vertretern anderer Gesundheitsberufe abgestimmt und entsprechend kommuniziert werden, werden häufig nicht umgesetzt. Gründe hierfür sind neben mangelnder Akzeptanz auf ärztlicher Seite Schwierigkeiten bei der Interpretation der Inhalte von Patientenverfügungen sowie organisatorische Schwierigkeiten bei der Umsetzung von Patientenverfügungen im hocharbeitsteiligen Gesundheitswesen. ACP umfasst einen kontinuierlichen Planungs- und Umsetzungsprozess für in der Zukunft liegende gesundheitsbezogene Entscheidungssituationen, in denen Patienten nicht selbstbestimmungsfähig sind. Die Bearbeitung konzeptioneller Fragestellungen zu ACP, Fortbildungen zur Umsetzung sowie die Messung der Effektivität von ACP mit Hilfe angemessener Endpunkte sind Aufgabenstellungen für weitere Forschungsarbeiten.
Abstract
Background
Advance directives are an expression of the patient’s will and should be taken into account when making decisions on behalf of patients with a lack of self-determination.
Objective
This article presents the concept of ACP and the effectiveness in healthcare with a focus on decisions in cancer care and discusses ACP from a clinical ethics perspective.
Method
Selective literature review in PubMed on publications in German and English on advance care planning.
Results and Conclusion
Experience in clinical practice indicates that advance directives which have not been discussed with and communicated among physicians or other members of healthcare teams are often not implemented. The lack of acceptance of advance directives by some physicians, challenges with the interpretation of advance directives and organizational difficulties when implementing advance directives into a healthcare system which is highly dependent on the division of labor, are the main reasons. Advance care planning (ACP) encompasses a continuous process in planning of the implementation of decisions on healthcare in the future for situations in which patients lack the capacity to make these decisions themselves. Conceptual aspects of ACP, educational interventions and the evaluation of the impact of ACP are tasks for further research.

Für „Advance Care Planning“ werden im deutschsprachigen Raum derzeit verschiedene Begriffe (z. B. „gesundheitliche Vorausplanung“, „vorausschauende Versorgungsplanung“ verwendet, die teilweise auch unterschiedlichen klinischen Settings geschuldet sind (z. B. hausärztliche Versorgung, Palliativmedizin). Aufgrund einer fehlenden überzeugenden Übersetzung des Begriffs verwenden wir in dieser Arbeit den englischsprachigen Ausdruck.
Der PSDA sieht vor, dass Patienten bei Aufnahme in Krankenhäuser und andere Gesundheitseinrichtungen über Patientenverfügungen und die Möglichkeit gesundheitsbezogener Vorausverfügungen informiert werden und weiterhin dokumentiert wird, ob eine Patientenverfügung vorliegt (Übersicht bei Vollmann u. Pfaff 2003).
