Hintergrund
Obwohl Krebserkrankungen bei älteren Menschen deutlich häufiger auftreten als bei jüngeren, erfuhr die psychoonkologische Betreuung älterer Patienten mit einer Krebserkrankung bisher nur wenig Beachtung.
Fragestellung
Welche Belastungsfaktoren sind typisch für ältere Krebspatienten? Welchen psychosozialen Versorgungsbedarf haben ältere Krebspatienten und welche spezifischen psychotherapeutischen Verfahren stehen für die Versorgung älterer Krebspatienten zur Verfügung?
Material und Methoden
Dieser Beitrag gibt einen Überblick über den aktuellen Forschungsstand.
Ergebnisse
Altersspezifische Belastungsfaktoren sind vor allem Gebrechlichkeit, komorbide körperliche Erkrankungen, kognitive Defizite sowie Einschränkungen bei der Alltagsbewältigung. Altersspezifische Unterstützungsangebote sollten möglichst einen niedrigschwelligen Charakter haben. Als altersspezifische psychotherapeutische Verfahren haben sich international vor allem narrative Interventionen etabliert, die auf Lebensrückblick und das Würdeerleben fokussieren wie beispielsweise die Reminiszenztherapie.
Diskussion
In Deutschland gibt es bisher kaum psychoonkologische Interventionen, die den spezifischen Bedürfnissen alter Krebspatienten Rechnung tragen. Auch die Wirksamkeit vieler psychosozialer Interventionen für ältere Krebspatienten ist bisher nur unzureichend erforscht.
Abstract
Background
Psycho-oncological care for older cancer patients has received little attention in research although the prevalence of cancer is significantly higher in this population than in younger people.
Objectives
Which stressors typically affect older cancer patients? Which psychosocial supportive care needs do older patients have and which specific psychotherapies are available to them?
Methods
This article gives an overview of the current state of research.
Results
The predominant age-specific stressors are frailty, physical comorbidities, cognitive deficits and limitations in activities of daily life. Age-specific supportive care should have as low a threshold as possible. Reminiscence therapy, life review therapy and dignity therapy are established age-specific forms of psychotherapy.
Conclusion
Few psycho-oncological interventions in Germany address the specific needs of older patients and more research regarding the effectiveness of such therapies is required.
