Zusammenfassung
Die Identifikation der Krebskranken, die so belastet sind, dass sie psychoonkologischer Unterstützung bedürfen, ist ein altes, immer noch nicht gelöstes Problem der Psychoonkologie. Dies hat damit zu tun, dass sich weder in der Forschung noch in der Praxis Kriterien und Maßnahmen für die Auswahl der Patienten herauskristallisiert bzw. bewährt haben. In Frage kommen die Kriterien Krankheits-/Behandlungsmerkmale, die Selbstzuweisung des Patienten, psychiatrische Störungen, Fremdeinschätzung des behandelnden Onkologen, Coping und psychosoziale Belastung/Lebensqualität.
Nationale und internationale Leitlinien empfehlen das systematische Screening aller Patienten mit geeigneten Instrumenten. Hierbei handelt es sich um bereits vorliegende Fragebogen. Die Realisierung dieser Vorgehensweise bleibt jedoch bisher auf wenige Zentren beschränkt und ist nicht Gegenstand der Versorgungsroutine. Ein Grund ist der relativ große Aufwand, der mit einem Einsatz von sog. Papier- und-Bleistifttests in der Praxis verbunden wäre. Moderne computergestützte Diagnostiksysteme können hier Abhilfe schaffen und so zu einer Verbesserung und Integration der Versorgung und zur Steigerung der Lebensqualität der Patienten beitragen.
Abstract
The systematic screening for distress in cancer patients is still an unsolved problem in pychooncology. One of the reasons is that there has been no agreement about the adequate criteria to select the patients (characteristics of disease or treatment, self-selection of patients, psychiatric comorbidity, coping, assessment of oncologist or cancer-specific distress and quality of life). National and international guidelines for psychosocial care recommend systematic screening of all cancer patients. However, distress screening programs have remained restricted to some cancer centers until now. One reason is the effort required to implement paper and pencil tests into medical routine. Using modern touch screen computers may help to solve the problem and hence improve the quality of medical treatment.
