Zusammenfassung
In Deutschland wurden 1999 wegen primärer oder sekundärer bösartiger Neubildungen in der Lunge 357.237 Fälle behandelt. Die häufigsten zur Notfallbehandlung führenden Symptome sind Hämoptysen, Dyspnoe und Pneumonien. Ursächlich für diese Symptome sind unter anderem endobronchiale Tumorarrosionen, bronchovaskuläre Fisteln, Obstruktion der Atemwege durch endo-, intra- oder extrabronchiales Tumorwachstum, maligne Pleuraergüsse und tracheoösophageale Fisteln. In der Akutsituation ist das Freihalten der Atemwege und die Sauerstoffgabe in der Regel ausreichend. Für die differenzierte Therapie werden die Laser- oder Elektrokoagulation zur Blutstillung, die Kombination von mechanischer Tumorreduktion und Lasertherapie zur Desobliteration, die Stentimplantation zur Sicherung und Wiederherstellung der Atemwege und die Dränage und Pleurodese des Pleuraraums eingesetzt. Operative Maßnahmen sind nach Stabilisierung des Patienten und entsprechender Diagnostik in kurativer und bei konservativ nicht zu behandelnden Problemen auch in palliativer Absicht angezeigt.
Abstract
In Germany 357,237 cases of primary or secondary malignancies of the lung were treated in 1999. The most common symptoms leading to emergency admission to a hospital are hemoptysis, dyspnea, and pneumonia. These symptoms are caused by tumor arrosion, bronchovascular fistula, malignant bronchial obstruction, malignant pleural effusion, and tracheoesophageal fistula. Usually clearing of the tracheobronchial tree and supplementation with oxygen is sufficient initial treatment. Further treatment to stop bleeding is possible with laser and electrocoagulation. To disobliterate the bronchial system, mechanical means and laser coagulation are appropriate. Stenting reexpands bronchial stenosis. Malignant pleural effusion commonly is treated by drainage and pleurodesis. Surgical therapy is indicated when the patient is stable and further investigation shows no contraindication. If conservative therapy is not able to solve the problem, surgery is indicated for palliation.
