Summary
Odontogenic infections represented a challenge for every craniomaxillofacial surgeon in the pre-antibiotic era, not least due to the frequent association with septic complications and lethal consequences. Nowadays, the incidences of serious infection-related complications are less frequent thanks to the medical progress. Nevertheless, the development of severe infections is still hard to foresee.
The following analysis provides an overview of four patients with severe, life-threatening odontogenic deep neck space infections treated at the University Clinic for Oral and Maxillofacial Surgery in Innsbruck and describes the etiology, therapy, and outcome for each patient while analyzing differences between the single cases. Crucial aspects in origin, progression, and treatment of DNIs are highlighted, comparing the results with the international literature.
Zusammenfassung
In der prä-antibiotischen Ära war die Behandlung schwerer dentogener Infektionen eine Herausforderung für jeden Mund-, Kiefer- und Gesichtschirurgen, nicht zuletzt aufgrund der häufigen septischen Komplikationen mit Todesfolge. Dank des medizinischen Fortschrittes hat die Häufigkeit infektionsbedingter Komplikationen deutlich abgenommen. Nichtsdestotrotz ist der Verlauf ausgedehnter Infektionen schwer vorherzusehen.
Die vorliegende Arbeit gibt einen Überblick über vier Patienten, die aufgrund lebensbedrohlicher dentogener Abszedierungen im Bereich der tiefen Halslogen an der Univ.-Klinik für Mund-, Kiefer- und Gesichtschirurgie in Innsbruck behandelt wurden. Die Ätiologie, Therapie und die Folgen des Krankheitsverlaufes werden beschrieben und Unterschiede zwischen den einzelnen Fällen analysiert. Grundlegende Aspekte in den Ursachen, im Verlauf und in der Behandlung schwerer dentogener Abszedierungen werden besprochen und mit Ergebnissen in der internationalen Literatur verglichen.
