Zusammenfassung
Hintergrund
Die Verwendung der Klassifikationskriterien des Amerikanischen Kollegiums für Rheumatologie (ACR) zur klinischen Diagnose des Fibromyalgiesyndroms (FMS) ist umstritten. Die Regionale Schmerzskala (RPS) wurde zur Diagnose des FMS in der Praxis und in epidemiologischen Studien entwickelt (Survey-Kriterien). Bislang wurde die RPS nicht im deutschsprachigen Raum validiert.
Methoden
216 Patienten mit FMS, 53 mit einer aktiven rheumatoiden Arthritis (RA) und 60 mit einer depressiven Störung aus klinischen Einrichtungen füllten die RPS aus; 43 Patienten mit FMS füllten die RPS innerhalb von 8 Wochen ein zweites Mal aus.
Ergebnisse
Der Intraclass-Koeffizient der Survey-Diagnose des FMS nach 8 Wochen war 0,78 (Test-Retest-Reliabilität). Von Ärzten wurde das 7-Tage-Kriterium des Schmerzes, von Patienten das Fehlen der Ankreuzmöglichkeiten einiger artikulärer Schmerzorte kritisiert (Augenscheinvalidität). Die Konkordanzrate der FMS-Diagnose nach den Survey- und den ACR-Kriterien betrug 84,7% (±1,1) (konvergente Validität). Die Survey-Kriterien des FMS (diskriminative Validität) erfüllten 45,3% der Patienten mit RA und 38,3% der Patienten mit depressiver Störung.
Schlussfolgerung
Die RPS weist eine gute Reliabilität und konvergente Validität, jedoch eine eingeschränkte diskriminative Validität auf. Die RPS kann als Sreeninginstrument zur Diagnose eines FMS in der Praxis benutzt werden.
Abstract
Objective
Use of the American College of Rheumatology classification criteria for the clinical diagnosis of fibromyalgia syndrome (FMS) is under debate. The Regional Pain Scale (RPS) had been developed for the diagnosis of FMS in clinical practice and in survey settings (survey criteria of FMS). So far a German version has not been validated.
Methods
A total of 216 patients with FMS, 53 with active rheumatoid arthritis (RA) and 60 with depressive disorder, recruited from clinical institutions completed the RPS. Forty-three patients with FMS filled in the RPS within 8 weeks a second time.
Results
The intraclass coefficient of FMS diagnosis after 8 weeks was 0.78 (test-retest reliability). The 7-day interval of pain was criticized by physicians. The absence of some articular pain regions was criticized by patients (face validity). The concordance of FMS diagnoses to survey and ACR criteria was 84.7% (±1.1); 45.3% of the patients with RA and 38.3% of the patients with depressive disorder met the survey criteria of FMS (discriminant validity).
Conclusion
The RPS has good reliability and convergent validity, but limited discriminant validity. The RPS is suited as a screening instrument for the diagnosis of FMS in clinical practice.
