Zusammenfassung
Bereits 1895 beschrieb Freud einen Fall von Anorexia nervosa als traumatisch bedingtes, hysterisches Phänomen. Im Jahr 1916 veröffentlichte Abraham seine wichtige Arbeit über die Beziehung zwischen oraler Fixierung und Depression. Bis in die späten 60er-Jahre betrachtete man die Bulimia nervosa als Spezialfall der Anorexie. Zu dieser Zeit waren die psychoanalytischen Vorstellungen über Essstörungen vor allem von der Triebpsychologie geprägt. Aber immer häufiger wurden dann zwei wichtige Beobachtungen gemacht: 1. Sehr häufig gelang es in den psychoanalytischen Behandlungen nicht, einen klassischen ödipalen Trieb-Abwehr-Konflikt festzustellen. 2. Es gab eine zunehmende Zahl von essgestörten Frauen mit Essanfällen und Erbrechen, jedoch ohne Untergewicht (ein Kriterium der Anorexia nervosa). So wurde in der Psychoanalyse die frühe bzw. präödipale Pathologie immer wichtiger. Diese Entwicklung war ganz wesentlich von Hilde Bruch beeinflusst. Gleichzeitig berücksichtigten die medizinischen Klassifikationen, dass es sich bei der Bulimia nervosa um ein eigenes Krankheitsbild handelt, das nicht mit der Anorexia nervosa identisch ist. Leider ist die Zahl von empirischen psychoanalytischen Studien über die Bulimie sehr klein und stattdessen werden oft Generalisierungen auf der Basis von Einzelfallberichten gemacht. In dieser Arbeit wird ein Behandlungsabschnitt einer bulimischen Patientin mit erheblicher narzisstischer Pathologie vorgestellt. Bei dieser Behandlung schienen selbstpsychologische Überlegungen genauso wichtig wie triebpathologische Erwägungen und Gedanken über die Objektbeziehungen. Es wird vorgeschlagen, diese Konzeptionen nicht alternativ, sondern gleichzeitig miteinander zu benutzen. Außerdem wird empfohlen, das klassische Setting zugunsten der Herstellung einer hilfreichen Beziehung zu modifizieren.
Abstract
Freud described already in 1895 a case of anorexia nervosa as a traumatic hysterical phenomenon. In 1916 Abraham published his important paper about the relationship between oral fixation and depression. Until the late sixties bulimia nervosa was considered to be a special symptomatology of anorexia nervosa. At that time psychoanalytic ideas about eating disorders depended largely on early drive psychological considerations. But more and more, two important observations were made: 1. In psychoanalytic treatments very often it was impossible to detect the classic oedipal conflict between drive and defense. 2. There was an increasing number of eating disordered women with the symptomatology of binge eating and vomiting without underweight (a criterion of anorexia nervosa). Thus, in the field of psychoanalysis the early or preoedipal pathology became more important. This development was mainly influenced by H. Bruch. At the same time the medical classifications took into account that bulimia nervosa has to be regarded as a separate disease (and not equivalent to anorexia nervosa). Unfortunately, the number of empirical psychoanalytic studies on bulimia nervosa is rather small and instead often generalizations are made on the basis of single case reports. This paper presents a part of the treatment of a female bulimic patient with a considerable narcissistic pathology. For this treatment self psychological considerations seemed to be important as well as drive pathological reflections and the idea of object relations. It is proposed not to use these conceptions alternatively but in conjunction. Moreover it is recommended to modify the classical setting to enhance the creation of a helpful alliance.
