Zusammenfassung
Unter Reproduktionsmedizinern besteht weitgehend Konsens dahingehend, dass Mehrlingsschwangerschaften aufgrund vielfältiger prä- und perinataler Risiken vermieden werden sollten. Dennoch sind 30–40 % aller Kinder nach assistierter Reproduktion Mehrlinge. Wenig geläufig sind die psychosozialen Konsequenzen für die betroffenen Familien, die weit über eine erhöhte Arbeitsbelastung im ersten postnatalen Jahr hinausreichen. Langfristig wirksame Folgerisiken der Mehrlingsgeburt ergeben sich aus persistierenden depressiven Störungen bei den Müttern, Belastungen der Paarbeziehung, einer quantitativ und qualitativ veränderten Eltern-Kind-Interaktion und einer erschwerten Individuation der Kinder im Kontext der Mehrlingsgeschwisterschaft. Frühgeburtlichkeit und ein Geburtsgewicht < 1.500 g gehen vermehrt mit Beeinträchtigungen der kognitiven und sozialen Entwicklung einher, die sich erst im Schulalter manifestieren können. Im Folgenden Beitrag werden in einem Literaturüberblick somatische, psychosoziale und emotionale Risiken der Mehrlingsentwicklung dargestellt und in ihrem Zusammenspiel betrachtet.
Abstract
Professionals in the field of reproductive medicine generally agree that multifetal pregnancies should be avoided because of increased pre- und perinatal risks. Nevertheless, 30–40 % of all children conceived through assisted reproductive techniques are multiples. There is insufficient awareness of the psychosocial consequences for multiple birth families that extend far beyond the caregiver's workload during the first postnatal year. Long-term risks derive from persistent maternal depression, fraught relationships between parents, a different mode of interaction in parent-child relationships and problems of individuation in multiples. Prematurity and very low birthweight are associated with impaired cognitive and social development that may not become evident until mid childhood. This article reviews the literature on the medical, psychosocial and emotional impact of multiple births on the families and addresses the risks for the well-being of the children and their parents.
