Summary
Transesophageal echocardiography (TEE) is a relatively non-invasive diagnostic and monitoring tool allowing an accurate on-line description of cardiovascular performance in the perioperative setting. The images produced not only yield information about the anatomy and structural integrity of the heart and great vessels but also offer continuous and accurate information about ventricular performance, ventriculo-arterial coupling and the different determinants of ventricular function. The lack of interference with the surgical field allows for rapid insertion during the perioperative course with prompt recognition and definition of the causes of hemodynamic disturbances and assessment of the effectiveness of the therapeutic measures taken.
In critically ill patients, sequential investigations can guide therapeutic interventions after obtaining a baseline cardiovascular profile of the patient and allow for an integrated approach to the hemodynamic problems.
A comprehensive evaluation of left ventricular function includes measurement of the different determinants of ventricular performance: preload, contractility and afterload. Transesophageal echocardiography is able to measure those individual determinants and the integration of the obtained information leads to a better understanding of the nature of hemodynamic disturbances.
Zusammenfassung
Transoesophageale Echokardiographie (TEE) zusammen mit der Dopplersonographie ermöglicht eine rasche Differentialdiagnose von haemodynamischen Störungen in Anästhesie und Intensivmedizin. Die globale linksventrikuläre Funktion und deren Determinanten Preload, Afterload und Kontraktilität sind sofort bestimmbar. Bei der Bestimmung des linksventrikulären Preloads liegt die Stärke des TEE in einer Integration der Information bezüglich Druck und Volumen. Unter Zuhilfenahme verschiedener Darstellungsmodalitäten kann die Annahme einer konstanten Beziehung zwischen Druck und Volumen während der Diastole umgangen werden. Die Afterloadbestimmung im TEE ist ebenfalls mit verschiedenen Methoden durchführbar. Dabei werden Veränderungen in ihren Effekten quantitativ und qualitativ erfasst. Die Wandeigenschaften des arteriellen Systems lassen sich charakterisieren. Die Bedeutung für das Verständnis verschiedener Krankheiten wie arterieller Hypertonie oder beim Marfansyndrom darf nicht unterschätzt werden. Neben der Powerbestimmung werden auch die klassischen Indizes zur Kontraktilitätsmessung erläutert.
