Summary
Amiodarone (Am) has emerged as an important drug for the treatment of cardiac arrhythmias. Its clinical use is increasing as evidence mounts as to its utility for a variety of forms of atrial and ventricular arrhythmias. Intravenous amiodarone, like the oral form, has been available for many years. Whether Am improves the rate of successful resuscitation after out-of-hospital cardiac arrest was studied recently. Five-hundred and four patients (pts) were included in a randomized, double-blind, placebo-controlled study of intravenous Am (300 mg i.v.) (246pts) or placebo (Pl)(258 pts). There were no significant clinical differences between the Am and the Pl groups. Of the 504 pts, 197 pts (39%) survived to be admitted to the hospital. Receipients of Am were more likely to survive to be admitted to the hospital (44%) compared to Pl (34%) (p=0.03). The benefit of Am was consistent among all subgroups. Therefore, it seems that Am is an important antiarrhythmic agent in pts with out-of-hospital cardiac arrest, in whom electrical cardioversion/defibrillation is unsuccessful. However, Am is not without risk and complications; therefore, surveillance of the pts is necessary.
Zusammenfassung
Amiodaron (Am) ist ein wichtiges Notfallmedikament zur Behandlung von Herzrhythmusstörungen. Es wird im klinischen Alltag bei einer Vielzahl supraventrikulärer und ventrikulärer Arrhythmien eingesetzt. Die intravenöse Applikationsform ist wie die orale Gabe seit vielen Jahren verfügbar. Ob Am zu einer Verbesserung des Reanimationserfolges bei Patienten (Pt) mit Herz-Kreislauf-Stillstand führt, wurde kürzlich untersucht. 504 Pt wurden in eine randomisierte, doppel-blinde, plazebo-kontrollierte Studie eingeschlossen; 246 Pt erhielten Am (300 mg i.v.) und 258 Pt wurden mit Plazebo (Pl) behandelt. Es gab keine signifikant klinischen Unterschiede zwischen beiden Gruppen. 197/504 Pt (39%) erreichten lebend eine Klinik; Pt, die Am erhielten, konnten wesentlich häufiger (44%) einer Klinik zugeführt werden als bei Pl Applikation (34%) (p=0,03). Der Nutzen von Am wurde auch in allen Untergruppen nachgewiesen. Am ist daher ein wichtiges Antiarrhythmikum für Pt mit Herz-Kreislauf-Stillstand, bei denen eine elektrische Kardioversion/Defibrillation nicht erfolgreich ist. Dennoch ist eine Am Applikation nicht ohne Risiken und Komplikationen; Pt müssen daher bei einer Am-Therapie sorgfältig überwacht werden.
