Early defibrillation in Europe

Summary

The termination of ventricular fibrillation by early defibrillation has high priority. Therefore, the various programs for early defibrillation were compared country by country. Target groups of those programs include besides physicians, medical and ambulance personnel, most importantly non-medical persons who are planned to be included by public access programs. The starting qualifications of these groups are totally different. Furthermore, the individual programs differ in the time effort for the training. In Europe, programs for the use of automated external defibrillators exist only occasionally. Reasons for this are the lack of open-mindedness for this method, lack of knowledge, logistic and legal problems. In some countries, law does not cover the use of defibrillators by non-medical persons or the use is differently restricted country by country. In many countries, the ambulance personnel has the competence to carry-out the preclinical defibrillation. Nevertheless, the qualifications of the ambulance personnel regarding reanimation are very dissimilar. We agree with the recommendations of the ERC regarding the proposed requirements for the use of AEDs in Europe.

Zusammenfassung

Die Beendigung eines Kammerflimmerns durch eine frühe Defibrillation ist ein mit hoher Priorität anzustrebendes Ziel. Aus diesem Grunde wurde ein länderübergreifender Vergleich der einzelnen Frühdefibrillationsprogramme vorgenommen. Zielgruppen für derartige Programme sind neben Ärzten und medizinischem und rettungsdienstlichem Fachpersonal vor allem Laien, die im Rahmen von Public-Access-Programmen einbezogen werden sollen. Die verschiedenen Gruppierungen haben vollkommen differente Ausgangsqualifikationen. Ebenso unterscheiden sich die einzelnen Programme im zeitlichen Umfang der Ausbildung. In Europa sind Programme zum Einsatz automatisierter externer Defibrillatoren nur lückenhaft vorhanden. Gründe hierfür sind fehlende Aufgeschlossenheit und Bekanntheit des Verfahrens, sowie organisatorische und juristische Probleme. In einigen Ländern gibt es keine gesetzliche Absicherung für die Anwendung von Defibrillatoren durch Laien. Darüber hinaus bestehen länderdifferente Beschränkungen. In vielen Ländern fällt die Durchführung der Erstdefibrillation in den Zuständigkeitsbereich des Rettungspersonals, das allerdings gleichfalls vollkommen verschiedene Qualifikationen im Hinblick auf eine Reanimation hat. Es besteht Übereinstimmung bezüglich der zu erhebenden Forderungen mit den Empfehlungen des ERC zur Anwendung von AEDs in Europa.
