Summary

In the course of sepsis, antithrombin III (AT) operates as an important inhibitor of the ensuing activation of coagulation, and additionally as a regulator of the inflammatory response. To be available for the latter function, AT has to bind heparin-like receptors on endothelial and mononuclear cells and on leukocytes. This binding is blocked by exogenously applied heparin. In the KyberSept clinical sepsis trial including more than 2000 patients, mortality was significantly lowered by AT in the group not treated with concomitant heparin.

Zusammenfassung

Bei Sepsis erweist sich Antithrombin III (AT) nicht nur als wichtigster Inhibitor der dramatischen Gerinnungsaktivierung, sondern auch als Regulator der Entzündungsreaktion. Voraussetzung ist allerdings die Bindung von AT an heparinähnliche Rezeptoren der Endothelien, Leukozyten und Monozyten. Standard- aber auch niedermolekulares Heparin, blockieren diese Bindung. In einer klinischen Sepsis-Studie (KyberSept) mit mehr als 2000 Patienten hat AT die Letalität nur in der Patientengruppe signifikant gesenkt, die nicht gleichzeitig Heparin erhalten hatte.
