Summary
Thoracic x-ray is an important part of daily routine in intensive care units. The developing radiation load causes great uncertainty. In the present study we made measurements at different distances, angles and heights around a phantom and patients. Measurements showed a local dose of 0.1 μSv per shot in a lateral distance of 1.5 m corresponding to the dose of a one hour lasting stay in the open. The 3300 thoracic x-rays per year taken in our intensive care unit caused a dose of 0.66 mSv at a distance of 1 m from patient without lead protection. This dose is clearly lower than the threshold value of 20 mSv/y for staff exposed to radiation. The dose of 0.66 mSv is so low that the threshold value of 1 mSv for pregnant woman is kept. The neighbouring patient is exposed to a dose of 0.6 μSv during a stay of 4 days in intensive care unit. The local radiation dose is low; the staff must not leave the room. At a distance of 3 m the dose amounts to 0.04 μSv corresponding to a stay in the open with a duration of 10 minutes.
Zusammenfassung
Röntgenthoraxaufnahmen gehören zur täglichen Routine auf Intensivstationen. Über die dabei auftretende Strahlenbelastung herrscht häufig große Unsicherheit. In der vorliegenden Studie wurden zur Ermittlung der Ortsdosis Messungen am Phantom und Patienten in verschiedenen Abständen, Winkeln und Höhen durchgeführt. Die Messungen zeigten, dass die Ortsdosis mit ca. 0,1 μSv pro Aufnahme im seitlichen Abstand beträgt und entspricht damit der Strahlenbelastung eines einstündigen Aufenthaltes im Freien. Bei den ca. 3300 auf der Station durchgeführten Thoraxaufnahmen/ a beträgt die Ortsdosis in einem Abstand von einem Meter vom Patienten ohne Bleischürze 0,66 mSv und liegt damit deutlich unter den geforderten 20 mSv/a für strahlenexponierte Beschäftigte. Die Dosis von 0,66 mSv ist so niedrig, dass sogar der Grenzwert von 1 mSv für Schwangere eingehalten wird. Für Nachbarpatienten ergibt sich bei einer durchschnittlichen Verweildauer von 4 d eine Dosis von 0,6 μSv.
Die gemessenen Ortsdosen sind niedrig, so dass es nicht notwendig ist den Raum zu verlassen, da im Abstand von 3 m die Ortsdosis bei 0,04 μSv liegt, entsprechend einem 10-minütigem Aufenthalt im Freien.
