Zusammenfassung
Hintergrund: Die Ergebnisse der CRYO-ROP-Gruppe belegen den Nutzen der Kryokoagulation der Netzhaut für Funktion und Morphologie bei Schwellenwert-Frühgeborenen-Retinopathie (RPM). Untersucht wird die Sehschärfe im Alter von 2 Jahren korrigierten Gestationsalters aller 48 Augen, die wegen akuter RPM zwischen 1990 und 1994 konsekutiv in unserer Klinik kryokoaguliert wurden.
Patienten und Methode: Sehschärfe und Fundusbefund wurden mittels Teller-Acuity-Cards-Test, Skiaskopie, orthoptischem Status und Funduskopie gemessen. Eine günstige Gittersehschärfe wurde festgelegt auf ≥ 2.2 Per/°, ein günstiger Fundusbefund als unauffälliger hinterer Pol bzw. Stadium 4 a.
Ergebnisse: 56 % aller Augen erreichten eine quantifizierbare Sehschärfe zwischen 0.32–13.0 Per/°, 42 % eine günstige Sehschärfe, 69 % der Augen zeigten einen günstigen Fundusbefund. 55 % der meßbaren Augen waren myop, 29 % hoch myop ≥–6.0 dpt. 47 % wiesen eine Heterotropie auf.
Schlußfolgerungen: Bezüglich eines günstigen Ergebnisses findet sich eine Diskrepanz zwischen Morphologie und Funktion. Insbesondere für Zone-1-Augen scheint die Kryokoagulation der Netzhaut nicht auszureichen, um ein sowohl morphologisch als auch funktionell günstiges Ergebnis zu erreichen.
Summary
Background: The results of the Cryo-ROP group showed that cryotherapy significantly reduces the rate of adverse morphological and functional outcome of eyes with threshold retinopathy of prematurity. Structural status and visual acuity of 48 eyes which consecutively underwent cryocoagulation in our hospital between 1990 and 1994 were examined at 2 years corrected gestational age.
Method: Visual acuity and fundus morphology were examined by Teller acuity-cards test, retinoscopy, orthoptic status and fundus examination. A favourable grating acuity was defined as ≥ 2.2 cy/°, a favourable morphology as no pathological or mild findings at the level of the posterior pole or stage 4 a.
Results: Fifty-six % of cases reached a quantifiable grating acuity between 0.32 and 13.0 cy/°, 42 % reached a favourable functional outcome, and 69 % of the eyes had a morphologically favourable outcome. Fifty-five % of the measurable eyes were myopic, and 29 % had a high myopia ≥–6.0 D. 47 % showed a heterotropia.
Conclusions: Our results show a discrepancy between morphological and functional outcome. Especially for zone 1 eyes, cryocoagulation of the avascular retina does not seem to allow a favourable morphological and functional outcome.
