Summary
Background: The success rate of combined glaucoma and small incision cataract surgery is not yet established.
Patients and methods: Therefore, 56 eyes of 52 patients (mean age 79.0 years) having trabeculectomy combined with phacoemulsification and implantation of a silicone posterior chamber lens were retrospectively investigated.
Results: The mean follow-up was 21.6 weeks. Due to the combined surgery the mean intraocular pressure decreased significantly (p < 0.001) from 21.8 to 14.8 mmHg. Seventy-five percent of the patients did not need any antiglaucomatous medication, while 25 % still had to use a local medication, but less frequently. The overall visual acuity increased significantly (p < 0.001) from a preoperative value of 0.24 to 0.52 postoperatively.
Conclusions: The results of combined glaucoma and cataract surgery seem as good as those reported for two separate glaucoma and cataract procedures.
Zusammenfassung
Fragestellung: In dieser Studie soll die mittelfristige Augendruckregulierung nach kombinierter Glaukom- und Katarakt-Operation überprüft werden.
Patienten und Methode: Bei 56 Augen von 52 Patienten im Alter von 56–90 Jahren (im Mittel 79,0 Jahre) mit fortgeschrittenem Glaukom und gleichzeitiger Katarakt wurde eine gedeckte Trepanation kombiniert mit Phakoemulsifikation und Implantation einer Silikon-Hinterkammerlinse durchgeführt.
Ergebnisse: Die Nachbeobachtungszeit betrug im Mittel 21,6 Wochen. Der Augeninnendruck, der präoperativ unter optimaler Therapie bei 21,8 mmHg lag, konnte hoch signifikant (p < 0,001) auf 14,8 mmHg im Mittel gesenkt werden. Bei 75 % der Patienten war postoperativ keine augendrucksenkende Therapie mehr nötig. Bei den restlichen 25 % konnte die Anzahl der Augentropfen deutlich reduziert werden. Der Visus stieg von präoperativ 0,24 (+/− 0,15) auf postoperativ 0,52 (+/− 0,25). Der Visusanstieg ist ebenfalls hoch signifikant (p < 0,001).
Schlußfolgerung: Durch einen kombinierten Eingriff kann sowohl die Glaukomsituation als auch der Visus verbessert werden, so daß gerade bei älteren Patienten diese Operation eine sinnvolle Alternative zum zweizeitigen Vorgehen darstellt.
