Ziel
Zahlreiche Verfahren gestatten zwar die geometrische Vermessung der Hornhaut als diagnostischen und therapeutischen Parameter, aber bisher blieb der biomechanische Zustand der Hornhaut (z. B. bei der refraktiven Hornhautchirurgie) unberücksichtigt. Ein weiteres starkes Interesse an der Biomechanik der Hornhaut entwickelte sich in den letzten Jahren von Seiten des Glaukommanagements. Zum einen beeinflussen neben den geometrischen Eigenschaften die biomechanischen Eigenschaften der Hornhaut die applanatorische Augeninnendruckmessung. Zum anderen konnten epidemiologische Studien zeigen, dass die Hornhautdicke ein eigenständiger Risikofaktor für die Entstehung und Progression des Glaukoms ist.
Methode
Mit dem Ocular Response Analyzer (ORA) steht erstmals eine Methode für die berührungslose Messung von biomechanischen Parametern der Hornhaut beim Patienten zur Verfügung. 2500 Messungen der kornealen Hysterese (CH) und des kornealen Widerstandsfaktors (CRF) wurden ausgewertet und der Einfluss von Hornhautdicke, intraokularem Druck sowie von strukturellen und pathologischen Veränderungen untersucht.
Ergebnisse
Es werden das Messprinzip, die Reproduzierbarkeit, der Unterschied zwischen statischen und dynamischen Messungen und deren Aussagefähigkeit erläutert. Des Weiteren werden Einflussfaktoren wie Hornhautdicke, intraokularer Druck, Alter und Quellung auf CH, CRF und CH-CRF quantitativ dargestellt. Für zahlreiche postoperative Strukturveränderungen an der Hornhaut (Lasik, Vernetzung, Keratoplastik) und auch für eine Reihe von pathologischen Veränderungen (Keratokonus, Hornhautdystrophie) werden die gemessenen CH- und CRF-Werte diskutiert. Die Veränderungen der CH und CRF bei pathologischen Zuständen werden anhand von strukturellen Veränderungen (Kollagen, GAG, Wassergehalt) erläutert. Einen besonderen Aspekt bildet die Bewertung von CH und CRF beim Glaukom.
Schlussfolgerungen
Die Messung der kornealen Hysterese und des kornealen Widerstandsfaktors der Hornhaut am Patienten ist ein wesentlicher Fortschritt für das Verständnis des biomechanischen Zustands der Hornhaut und besonders der pathologischen Veränderungen, z. B. bei Keratokonus und bei Glaukom.
Abstract
Aim
Several methods permit the measurement of geometric parameters of the cornea, but until now biomechanical conditions of the cornea have been ignored (e.g. in refractive corneal surgery). Besides the geometric condition, biomechanical properties of the cornea have been shown to influence applanation measurement of intra-ocular pressure (IOP) and epidemiological studies have identified corneal thickness as an independent risk factor for the development and progression of glaucoma. The aim of this investigation was to characterize the biomechanical properties of the cornea using the ocular response analyzer (ORA).
Methods
The ocular response analyzer (ORA) is a new method available for non-contact measurement of the biomechanical properties of the cornea. We evaluated the reproducibility of measurements, the difference between static and dynamic factors and the impact of independent factors (e.g. IOP, age, CCT, swelling of the cornea) on 2,500 measurements of corneal hysteresis (CH) and corneal resistance factor (CRF).
Results
In a large sample size we observed changes in CH and CRF after refractive surgery procedures (LASIK, UV-A cross-linking, keratoplasty) and in other corneal disorders (keratoconus, corneal dystrophies).
Conclusions
CRF and CH changes may reflect structural changes of the cornea. Thus, the ORA provides valuable information for a better understanding and characterization of the biomechanical condition of the cornea, especially with regard to diseases such as keratoconus and glaucoma.
