Zusammenfassung
Ziel der Studie war der Vergleich der Rotationsstabilität von Intraokularlinsen (IOL) mit C-Haptiken und solchen mit Z-Haptiken.
Prospektiv wurden 50 Kataraktpatienten zu gleichen Teilen auf die im Design äquivalenten IOL MS 612 S (C-Haptik) und MS 6120 (Z-Haptik) randomisiert. Klinische Endpunkte: IOL-Rotation nach einem Tag, einem Monat und 3 Monaten postoperativ. Zum Signifikanzvergleich der Gruppen wurde der unverbundene Wilcoxon-Test angewandt bei stetigen Endpunkten und für den binären primären Endpunkt der Studie („Rotation um mindestens 10°“) der exakte Fisher-Test.
In der Gruppe der C-Haptik ergab sich eine IOL-Rotation im Median von 0° am 1. Tag sowie von 2° mit dem Uhrzeigersinn nach 1 und 3 Monaten. In der Gruppe der Z-Haptik war die IOL am 1. Tag sowie nach 1 und 3 Monaten im Median um 0° rotiert. Tendenziell zeigten sich in der Gruppe der C-Haptiken größere Ausreißer (Spannweite nach 3 Monaten 0–21° bei C-Haptiken und 0–15,5° bei Z-Haptiken). Die Haptik-Gruppen unterschieden sich zu keinem postoperativen Zeitpunkt statistisch signifikant. Drei Monate postoperativ waren 32% der C-Haptik-IOL und 16% der Z-Haptik-IOL um mindestens 10° rotiert (p=0,32). Die Rotationsrichtung war bei C-Haptik-IOL zu 59% im Uhrzeigersinn, bei Z-Haptik-IOL rotierten 40% in diese Richtung (p=0,33).
Drei Monate postoperativ wiesen beide IOL eine gute Rotationsstabilität auf mit einer durchschnittlich nur minimalen Abweichung von der Implantationsachse. Es lässt sich im Langzeitverlauf eine tendenziell höhere Rotationsstabilität bei IOL mit Z-Haptik feststellen, die sich aber als nicht statistisch signifikant erwies. In der visuellen Rehabilitation zeigte sich nach 3 Monaten kein statistisch signifikanter Unterschied der Gruppen.
Abstract
The aim of the study was to compare the rotational stability of intraocular lenses (IOLs) with C-loop haptics and those with Z haptics.
A total of 50 patients with cataracts were prospectively randomised in equal numbers using a design equivalent to IOL MS 612 S (C-loop haptic) and MS 6120 (Z haptic, both HumanOptics, Erlangen). Complete ophthalmological examinations were performed including assessment of the IOL rotation after 1 day, 1 month and 3 months postoperatively. The significances between the groups were evaluated using the Wilcoxon test. The Fisher exact test was used for the primary finding of the study (rotation of at least 10°).
The IOL group with the C-loop haptics showed a median lens rotation of 0° on the first day and 2° clockwise after 1 month and 3 months. In the Z haptic group, there was no median rotation in the IOL group. Moreover, the range of IOL rotation of the C-loop haptics was broader (3 months postoperatively: maximum in the C-loop haptics: 21° with 15.5° in the Z haptics). There was no significant difference at any time. At 3 months postoperatively, 32% of the C-loop haptic IOL and 16% of the Z haptic IOL rotated at least 10° (P=0.32). In 59% of the IOL with C-loop haptics, the direction of the rotation was clockwise. This was 40% for the IOL with Z haptics (P=0.33).
At 3 months postoperatively, both IOLs demonstrated good rotational stability with a low mean deviation from the target axis. IOLs with Z haptics showed a tendency to greater rotational stability than C-loop haptics but without any significant difference. There was no significant difference in visual rehabilitation.
