Zusammenfassung
Für eine zielgerichtete Therapie der kraniomandibulären Dysfunktion (CMD) müssen deren potenzielle Auslöser spezifiziert und reproduzierbar diagnostiziert werden. Die Auslöser können entweder direkt im Kauorgan liegen oder mit einer Dekompensation der Körperstatik, einem Trauma oder einer systemischen Erkrankung einhergehen. Ziel der vorliegenden Pilotstudie ist es zu klären, ob auslösende Faktoren für eine CMD anhand eines modifizierten Schmerzfragebogens mit Schmerzlokalisierungszonen und numerischer Ratingskala (SL-NRS) zu erkennen sind und ob zwischen Haupt- und Nebenbeschwerden unterschieden werden kann. Anhand von Schmerzskalen wurden bei 103 Patienten mit CMD das allgemeine Schmerzempfinden sowie Schmerzen in 12 Körperregionen erfragt. Die Ergebnisse der Studie zeigen signifikant (p<0,0001), dass Schmerz nicht homogen wahrgenommen wird und Patienten den Primärschmerz spezifischen Körperregionen zuordnen konnten. Die erneute Abfrage der Schmerzintensität nach manueller Therapie im Kopf- und Kieferbereich zeigt ebenfalls signifikant positive Veränderungen (gepaarter t-Test; p<0,0001). Der Vorteil liegt in der Lokalisierung und Dokumentation von Schmerz und der Differenzierung, ob eine Schmerzursache unmittelbar im Kauorgan oder z. B. aufgrund einer Dekompensation in der Körperstatik zu finden ist.
Abstract
For targeted therapy of craniomandibular dysfunction (CMD) the potential trigger must be specified and reproducibly diagnosed. The trigger can be located in the masticatory apparatus or originate from a decompensated body posture, a traumatic event or a systemic disease. The aim of this pilot study was to clarify whether trigger factors for CMD can be recognized using a modified pain questionnaire with pain localization fields and a numeric rating scale (SL-NRS) and whether a differentiation between primary and secondary complaints can be made. A total of 103 patients were asked about their general sensitivity to pain as well as pain in 12 body regions using a pain scale. The results of the study showed at significant level (p<0.0001) that pain was not homogenously experienced and that patients could assign the primary pain to a specific body region. The renewed questions on pain intensity after manual therapy of the head and jaws also showed significant positive changes (paired t-test; p<0.0001). The questionnaire localized and documented pain and differentiated whether the cause of pain could be found directly in the masticatory oragn or in a decompensated body posture.
