Zusammenfassung
Gegenstand der manuellen Medizin ist die Untersuchung und Behandlung von Dysfunktionen am Bewegungssystem. Die zugrunde liegenden Wirkungsmechanismen werden einerseits in der Nozizeption und Schmerztherapie, aber auch in Störungen im Bewegungssystem mit vorwiegend neurophysiologischen Aspekten gesehen. In diesem Beitrag wird eine Übersicht über Vorgänge im tonischen motorischen Teilsystem gegeben, die bei störenden Einflüssen zu Muskelspannungsänderungen, Haltungsasymmetrien und Schmerzen führen. Nach einer Übersicht zu den tonischen Komponenten der Sensomotorik werden unter dem Aspekt der Neuroplastizität in der spinalen Motorik experimentelle Ergebnisse zur Sensibilisierung und Konditionierung an Motoneuronen und Interneuronen, zur Veränderung der Reflexerregung, zu supraspinaler Einflussnahme und zum Auftreten von Plateaupotenzialen zusammengestellt. Es wird postuliert, dass solche experimentell bestätigten neurophysiologischen Vorgänge beim Menschen zu Muskelspannungsänderungen, Haltungsasymmetrien und sekundär zu Schmerzen führen können. Diese Änderungen werden als Symptome in manualmedizinischen Untersuchungen erfasst. Gleichzeitig sind die gleichen neurophysiologischen Vorgänge im tonischen motorischen System auch Basis für die Erklärung der Wirkung der manuellen Therapie.
Abstract
Manual medicine investigates and treats dysfunctions in the motor system. The effective mechanisms are based on nociception and pain therapy as well as on disturbances in motor function under primarily neurophysiological aspects. This paper gives an overview of functions in the tonic part of the motor system, where disturbing stimuli cause changes in muscle tone, asymmetry in posture and pain. After a review of tonic components of the sensorimotor system, experimental results of sensitization and conditioning on motor neurons and interneurons are shown under the aspect of neuroplasticity in spinal motor control. Changes in reflex activation, the supraspinal influence and the existence of plateau potentials are also discussed. It is assumed that such experimentally proven neurophysiological processes may be a cause of muscle tension, asymmetry and secondary pain in humans. These alterations are symptoms covered in manual medical investigations. Simultaneously the same neurophysiological processes in the tonic motor system are also a basis for the explanation for the effects of manual therapy.
