Zusammenfassung
Neoadjuvante Therapieschemata als präoperative Radio- und/oder Chemotherapie vor der eigentlichen Tumorentfernung sind heute integraler Bestandteil interdisziplinärer Tumortherapien. Die neoadjuvante Therapie führt dabei zu therapieinduzierten Effekten auf die Tumorzellen. Zur Beurteilung dieser Tumorresponse, also des Therapieansprechens, sollte in der histopathologischen Diagnostik ein Regressionsgrading verwendet werden, das im Allgemeinen den Anteil des vitalen Resttumors im Verhältnis zum Gesamttumor beschreibt. Bis heute existieren keine einheitlichen Regressionsgraduierungssysteme. Die hier aufgeführten Graduierungssysteme stellen Empfehlungen dar, wobei das Regressionsgrading nach Dworak für Rektumkarzinome in die S3-Leitlinie für kolorektale Karzinome aufgenommen worden ist. Generell gelten für neoadjuvant therapierte Tumoren die allgemeinen Richtlinien der Aufarbeitung in der histopathologischen Diagnostik. Speziell wird jedoch in der Tumorklassifikation das Tumorgrading durch das Regressionsgrading ersetzt. Weiterhin werden neoadjuvant therapierte Tumoren in der TNM-Klassifikation durch das Präffix „y“ gekennzeichnet.
Abstract
Preoperative neoadjuvant chemoradiation therapy is a well-established and essential part of the interdisciplinary treatment of gastrointestinal tumors. Neoadjuvant treatment leads to regressive changes in tumors. To evaluate the histological tumor response different scoring systems describing regressive changes are used and known as tumor regression grading. Tumor regression grading is usually based on the presence of residual vital tumor cells in proportion to the total tumor size. Currently, no nationally or internationally accepted grading systems exist. In general, common guidelines should be used in the pathohistological diagnostics of tumors after neoadjuvant therapy. In particularly, the standard tumor grading will be replaced by tumor regression grading. Furthermore, tumors after neoadjuvant treatment are marked with the prefix “y” in the TNM classification.
