Zusammenfassung
Nachdem heute zunehmend mehr Patienten mit kolorektalen Karzinomen präoperativ multimodal therapiert werden, müssen die therapieinduzierten Veränderungen des Tumors nicht nur dem Pathologen bekannt sein, sondern auch bei Klassifikationen einheitlich berücksichtigt werden. Die Therapieeffekte können nach neoadjuvanter Therapie bei Patienten mit Rektumkarzinomen so ausgeprägt sein, dass in manchen Fällen trotz intensiver histopathologischer Aufarbeitung keine residuellen Tumorzellen nachweisbar sind. Es stellt sich die Frage, wie derartige Veränderungen klassifiziert werden und hinsichtlich ihrer biologischen Bedeutung eingeordnet werden müssen. Insbesondere sollten sie von spontanen, therapieunabhängigen Regressionsphänomenen abgegrenzt werden. Momentan werden regressive Veränderungen weder national noch international einheitlich bewertet, darüber hinaus unterschiedlich klassifiziert. In der TNM-Klassifikation wurde mit der 6. Auflage (2002) ein allgemein verbindlicher Klassifikationsvorschlag eingeführt, der ausschließlich vitale Tumorzellen berücksichtigt und daher wahrscheinlich gut reproduzierbar sein wird. Im Folgenden sollen die Vorschläge für kolorektale Karzinome kurz erläutert und diskutiert werden. Die Vorschläge basieren auf Erfahrungen mit Rektumkarzinomen.
Abstract
The treatment of patients with colorectal cancer has changed over the last decades. More and more patients receive combined modality treatments including (preoperative) radiotherapy and chemotherapy. To assess the colorectal cancer specimen after neoadjuvant therapy, the pathologist has to be familiar with the histological features induced by radiochemotherapy. Performing a standardized pathological procedure, different grades of tumour regression can be observed and tumour staging should be standardized using valid and reproducible criteria. These criteria are recommended in the forthcoming TNM classification (6th edition, 2002). In the review, these classification rules will be discussed for rectal cancer.
