Zusammenfassung
Das vorgestellte, empirisch abgesicherte Konzept zur störungsspezifischen Kurzpsychotherapie der Bulimia nervosa in 25 Sitzungen integriert kognitiv-behaviorale Ansätze und psychodynamisches Konflikt- und Therapieverständnis. Es wurde bei mehr als 200 unausgelesenen stationär oder ambulant behandelten Patientinnen1, in Einzel- und/oder Gruppentherapie, bei unterschiedlicher Sitzungsfrequenz von Ärzten, Psychologen, Pflegepersonal und Sozialpädagogen mit unterschiedlichen therapeutischen Orientierungen erfolgreich genutzt. Das Vorgehen und seine Ziele werden entlang den idealtypischen Behandlungsphasen (Vorbereitung, Einstieg, Hauptphase und Abschluß) skizziert und durch ein Fallbeispiel illustriert. Die therapeutischen Instrumente (Therapievereinbarung, Eßplan, Tagebuch, Wochenplan) und der Umgang mit ihnen in der therapeutischen Beziehung werden dargestellt. Besonders eingegangen wird auf die therapeutische Grundhaltung und auf Problembereiche wie Symptompersistenz, Widerstand und charakteristische (Gegen-) Übertragungen.
Summary
A research-based program for the disorder specific treatment of bulimia nervosa in 25 sessions is presented. It's effectiveness is well established. It provides the basis for major ongoing studies of therapy process and effectiveness. The concept was successfully applied to more than 200 unselected in- and outpatients in individual, group and combined therapies with high, low and flexible session frequency by doctors, psychologists, nurses and social workers of different theoretical orientation. It uses cognitive-behavioral techniques within a psychodynamic framework. The treatment is sequentially structured (preparatory, opening, main and closing stages). Therapeutic goals and procedures are specified for each stage. The therapeutic instruments (therapy, contract, diary, checklist, list of recommended carbohydrates) are described with special emphasis on their use within the therapeutic relationship. Special issues like persistence of symptoms, resistance and typical (counter-) transference reactions are discussed.
