Zusammenfassung
Der Obduktionsbefund intramuskulärer Hämorrhagien ist bei verschiedenen Todesursachen zu erheben. In primär unklaren oder zweifelhaften Fällen kann die histologische Untersuchung der betroffenen Skelettmuskulatur Hinweise auf den Entstehungszeitpunkt (antemortal versus postmortal), in einigen Fällen auch auf die Entstehungsart der Muskelblutungen geben. Wir berichten über den Fall eines psychiatrisch erkrankten Elektroingenieurs, welcher sich mittels einer selbst gefertigten Strom führenden Apparatur suizidierte. Autoptisch fanden sich ausgeprägte intramuskuläre Hämorrhagien in den im Stromweg gelegenen Muskelgruppen, deren Entstehungszeitpunkt durch die makromorphologische Befunderhebung allein nicht sicher zu klären war. Die histologische Untersuchung der mit PTAH gefärbten Skelettmuskulatur zeigte Hyperkontraktionsbanden, einen segmentalen und diskoiden Zerfall der Muskelfasern sowie eine pathologische Längsstreifung. Dieses vitale Schädigungsmuster lässt den Rückschluss auf eine antemortale Entstehung im Rahmen strominduzierter tetanischer Muskelkontraktionen zu. Zudem ließen sich histologisch weitere stromassoziierte Organveränderungen an Haut und Herzmuskulatur nachweisen.
Abstract
Intramuscular haemorrhages can be found in different causes of death. In some cases, the histological examination of the affected skeletal muscles can reveal the time of origin (antemortem versus postmortem) or even the underlying pathomechanism, especially with respect to the topographical distribution of the lesions. We report a case of an electrical engineer with a history of psychiatric problems who committed suicide by electrocution using a home-made device, consisting of a connecting plug, a pair of scissors and a magnifying glass. At autopsy, intramuscular haemorrhages were found in the skeletal muscles of the upper extremities and the muscles of the superior back, but the aetiology could not be determined by gross examination. The histological examination of PTAH-stained skeletal muscle specimens revealed hypercontraction areas, a segmental and discoid disintegration of the contractile apparatus and a pathological longitudinal striation. Due to the topographical distribution and micromorphological pattern of the skeletal muscle alterations indicating a vital origin of the haemorrhages, it was concluded that these were caused by tetanic current-induced muscle contractions. In addition, histological changes of the cardiac muscle and skin associated with electrocution could be detected.
