Zusammenfassung
Ziel forensisch-toxikologischer Untersuchungen an Obduktionsmaterial ist zu prüfen, ob Fremdstoffe direkt oder indirekt ursächlich für den Tod oder eine Einschränkung des Bewusstseins waren. Bereits präfinal ändern sich pharmakokinetische Parameter, weitere Einflussgrößen sind notärztliche Maßnahmen, Krankheit oder Toleranzentwicklung. Viele Xenobiotika unterliegen dem Phänomen der postmortalen Umverteilung. Insbesondere bei fortgeschrittener Fäulnis können Substanzen neu gebildet oder abgebaut werden; die Veränderungen während der Leichenliegezeit betreffen auch die Untersuchungsmatrix. Die Wahl der Asservate ist von der Fragestellung und ihrer Verfügbarkeit abhängig. Die Analysen unterliegen den für forensisch-toxikologische Untersuchungen empfohlenen Qualitätssicherungsmaßnahmen. Besonderheiten bei der Analyse betreffen die Extraktionsausbeute und Trennmethoden, die Zersetzungsprodukte mit erfassen sollten. Eine Interpretation kann nur fallbezogen erfolgen.
Abstract
The results from toxicological analysis in a death investigation are applied to determine whether foreign substances have caused or contributed to death or to impairment. Drug concentration is likely to change already during the pre-final phase due to altered pharmacokinetics, treatment during resuscitation or at the intensive care unit, illness, or presence of tolerance. The potential for post-mortem changes must be considered for all but a few drugs. Formation of new entities as well as degradation of drugs may occur, especially in putrefied corpses; in addition, body fluids and tissues are severely affected due to autolysis and putrefaction. Specimens are selected based on the individual case history and their availability. The analytical procedures are performed in accordance with a proper quality assurance program for toxicological investigations. Problems are most likely to occur during isolation and identification of a drug. Interpretation is limited to the information on the particular case.
