Zusammenfassung
Die klinischen Kriterien der Sepsisdiagnostik sind neu definiert und in das Konzept des „systemic inflammatory response syndrome“ (SIRS) integriert worden. Das neue Verständnis um die pathophysiologischen Grundlagen von Sepsis und SIRS eröffnet auch für die postmortale Diagnostik neue Möglichkeiten. Immunhistochemisch stellen der Nachweis von E-Selectin auf Endothelien der interstitiellen prä- und postkapillären Gefäßstrombahn und von Laktoferrin auf intravaskulären, interstitiellen und intraalveolären Leukozyten nicht zu unterschätzende Bausteine in der mikromorphologischen Diagnostik dar. Procalcitonin (PCT) erscheint unter den derzeit zur Verfügung stehenden biochemischen Inflammationsparametern aufgrund seiner hohen Sensitivität und Spezifität für Sepsis und wegen seiner ausgesprochenen Hämolyseresistenz, langen Halbwertszeit und guten Stabilität ex vivo als der aussichtsreichste biochemische Parameter für den routinemäßigen Einsatz in der postmortalen Sepsisdiagnostik.
Abstract
The clinical criteria for the diagnosis of sepsis have been newly defined and integrated into the concept of the systemic inflammatory response syndrome (SIRS). This new understanding of the pathophysiological basis of sepsis and SIRS gives also new insights into the postmortem diagnosis. Immunhistochemically, the detection of E-selectin on endothelium of the interstitial pre- and postcapillary vasculature and of lactoferrin on intravascular, interstitial and intraalveolar leukocytes are further components contributing to the micromorphological diagnosis. Out of the different parameters of inflammation available so far, procalcitonin (PCT) is the most promising biochemical parameter for routine use in the postmortem diagnosis of sepsis because of its high sensitivity and specifity, great resistance against hemolysis, long half-life and good stability ex vivo.
