Abstract
The HELLP syndrome is a severe and life-threatening complication of pre-eclampsia. Its management has been controversial, some authors recommending a conservative approach for induction of pulmonary maturity in the fetus when the pregnancy has not reached the 32nd (34th) week while the majority recommend immediate delivery by caesarean section in the case of an unfavourable cervix, regardless of gestational age. Aggressive management means early recognition of the condition by laboratory screening methods on admission and immediate delivery once the diagnosis has been confirmed. Patients with the HELLP syndrome should be treated in a perinatal centre. The high maternal mortality (3%) and morbidity (12.5–65%) and also the high perinatal mortality (22%) are due to unpredictable complications and the development of disseminated intravascular coagulation (DIC). It has been shown that the frequency of DIC increases significantly with lengthening time interval between diagnosis and delivery. As there are no reliable clinical or laboratory parameters that can be used to predict the course and prognosis of the disease, the outcome of the HELLP syndrome is not calculable. A review of the literature published since 1990 clearly shows that aggressive management of the HELLP syndrome is associated with a significant reduction in maternal and fetal mortality.
Zusammenfassung
Das HELLP-Syndrom stellt eine schwere, lebensbedrohliche Verlaufsform der Präeklampsie mit typischer laborchemischer Konstellation dar. Das geburtshilfliche Vorgehen wird kontrovers diskutiert. Einige Autoren empfehlen ein abwartendes, konservatives Vorgehen mit Induktion der fetalen Lungenreife bei Schwangerschaften unterhalb der 32. (34.) SSW, die Mehrzahl fordert eine rasche Schwangerschaftsbeendigung durch Sectio caesarea bei unreifer Zervix unabhängig vom Schwangerschaftsalter. Das aggressive Vorgehen beim HELLP-Syndrom besteht in einer raschen Diagnosestellung durch ein laborchemisches Screening bei Aufnahme der Patientin und in der unverzüglichen Schwangerschaftsbeendigung. Patientinnen mit HELLP-Syndrom sollten in einem perinatalen Zentrum behandelt werden. Die hohe mütterliche Letalität (3%) und Morbidität (12,5–65%) sowie die hohe perinatale Mortalität (22%) sind vor allem durch unvorhersehbar auftretende Komplikationen und durch die Entwicklung einer disseminierten intravasalen Gerinnung bedingt, deren Frequenz in Abhängigkeit von der zeitlichen Latenz zwischen Diagnosestellung und Entbindung zunimmt. Da bisher keine prädiktiven Parameter zu Verlauf und Prognose der Erkrankung vorliegen, ist der Verlauf des HELLP-Syndroms unkalkulierbar. Eine Literaturzusammenstellung seit 1990 zeigt, daß durch das aggressive Vorgehen beim HELLP-Syndrom die mütterliche und kindliche Mortalität deutlich gesenkt werden konnte.
