Im Rahmen einer retrospektiven Studie wurden 41 Patienten, bei denen im Kindesalter eine vordere Kreuzbandreinsertion durchgeführt worden war, durchschnittlich 6 Jahre nach der Operation nachuntersucht. Alle Kinder hatten zum Zeitpunkt der Reinsertion offene Epiphysenfugen. 37 Kinder mit proximal intraligamentären Kreuzbandrupturen wurden in der Technik nach Marshall operiert, 4 distale Kreuzbandrupturen wurden transossär reinseriert. Bei der klinischen und radiologischen Stabilitätsprüfung hatten 23 Patienten einen ++ oder +++ positiven Lachman-Test und bei 22 Patienten konnte ein Pivot Shift ausgelöst werden. Der durchschnittliche Lysholm-Score lag bei 91 Punkten, wobei die Funktionsminderung von 10% vom Grad der Instabilität unabhängig war. Die isokinetische Kraftmessung bei 32 Patienten ergab eine durchschnittliche Verminderung der Maximalkraft von 10%, wobei der Kraftverlust ebenfalls vom Grad der Instabilität unabhängig war. Aufgrund der schlechten objektiven Ergebnisse mit Instabilität bei über 50% der reinserierten Kreuzbänder kann die Kreuzbandreinsertion bei Kindern nicht empfohlen werden.
We undertook a retrospective evaluation of 41 children who underwent ACL repair. The mean follow-up period was 6 years. At the time of surgery all patients had open epiphyseal plates. Thirty-seven proximal midsubstance tears were operated in the Marshall technique. Four distal midsubstance tears were repaired with transosseous sutures through the tibial condyle. At follow-up, the ACL in 23 of 41 patients was evaluated as being extremely unstable. The clinical examination and using radiologic stability tests revealed in those 23 patients a ++ or +++ Lachman, and 22 patients had a pivot shift. The mean Lysholm score was 91 points; on average all patients had a functinal deficit of 10%, independent of the amount of their instability. Isokinetic quadriceps testing revealed in all groups a minus of 10% quadriceps muscle strength compared with the healthy contralateral limb. The repair of ACL tears in children showed unacceptable objective results. This procedure cannot be recommended because more than 50% of the children with ACL repairs had unstable knees at follow-up.
