Hintergrund
Die Technik eines „minimal-invasiven“ anterolateralen, intrakapsulären modifizierten Hardinge-Zugangs mit vom Seniorautor entwickelter transossärer Muskel-Kapsel-Refixation wird detailliert beschrieben.
Material und Methode
Klinische und radiologische Daten nach zementierter Versorgung zeigten adäquate Ergebnisse ohne erhöhte Komplikationsraten auch bei Inzisionslängen von durchschnittlich 10 cm. Das Verfahren kann auch durch Operateure in Ausbildung erlernt und in entsprechender tolerabler Operationszeit sicher angewendet werden.
Ergebnisse
Ob Unterschiede zu einer konventionellen Operationstechnik vorliegen, kann aus unseren Erfahrungen und Daten nicht abgeleitet werden, aber die publizierte Datenlage scheint den fast ausschließlich kosmetischen Benefit für die Patienten zu bestätigen. Viel entscheidender als eine kurze Inzision bleibt die technisch sauber durchgeführte Operation mit entsprechenden Langzeitresultaten.
Abstract
Background
The technique of a minimally invasive anterolateral, intracapsular, modified Hardinge approach with transosseous refixation developed by the senior author is described in detail.
Material and methods
Clinical and radiographic data after cemented total hip arthroplasty reveal adequate outcome without increased complication rates despite limited incision technique (average 10 cm). This technique can be safely applied by surgeons in training and performed in acceptable operating times.
Results
A comparison of the described technique to a standard incision cannot be made from our data, but current literature suggests the main benefit to be cosmetic. A technically well performed operation with adequate long-term outcome remains far more important than a short incision.
