Zusammenfassung
Der akute Gelenkinfekt ist ein seltenes Ereignis; die Folgen bei zu spät erkanntem Infekt jedoch teilweise dramatisch. Zur Diagnostik bieten sich vor allem das C-reaktive Protein sowie die diagnostische Gelenkpunktion an. Als therapeutische Maßnahme für den Frühinfekt haben sich arthroskopische Verfahren bewährt. Das arthroskopische Vorgehen mit Lavage und Débridement mit zusätzlicher systemischer Antibiotikagabe ist eine effiziente Technik zur Behandlung von septischen Gelenkinfekten. Finden sich anhaltende Infektzeichen auch unter antibiotischer Behandlung, so ist eine rasche Rearthroskopie innerhalb weniger Tage indiziert. Die Anzahl der notwendigen Rearthroskopien hängt von dem initialen Stadium des Infekts ab. Mit dem Shaver sollte ein sorgfältiges Débridement der nekrotischen Anteile und Verwachsungen erfolgen, ohne eine komplette Synovektomie der intakten Synovialmembran durchzuführen. Die Verwendung intraartikulärer resorbierbarer Antibiotikaträger ist möglich; diesem Vorgehen wird in der Literatur jedoch nur eingeschränkt zugestimmt. Saug-Spül-Drainagen sind nur noch sehr selten indiziert. Bei ossärer Mitbeteiligung ist die Arthrotomie immer noch Methode der Wahl.
Abstract
The acute joint infection is a rare condition; the delayed diagnosis may lead to significant joint destruction. Diagnostic tools are the c-reactive protein and joint aspiration. Today arthroscopic treatment options are suitable tools for the treatment of such conditions. Arthroscopic lavage and debridement with additional systemic antibiotic treatment proofed to be very effective in an infected joint. If there are continuous signs of infection even with antibiotic treatment, rearthroscopy within few days is indicated. The number of rearthroscopies is related to the initial stage of the infection. With a shaver thorough debridement of all necrotic tissue as well as resection of adhesions is performed without complete resection of the intact and noninfected synovial membrane. The use of intraarticular resorbable antibiotic fleece is possible, but not generally recommended. Suction irrigation systems are only rarely indicated. If there is bony involvement, open arthrotomy still is the golden standard.
