Zusammenfassung
Adipositas ist eine der häufigsten Gesundheitsstörungen der westlichen Industrieländer. Der Übersichtsartikel veranschaulicht die neuroendokrine Regulation der Nahrungsaufnahme sowie den Zusammenhang zwischen Übergewicht, generalisierter Inflammation und Störung der endokrinen und reproduktiven Funktionen der Frau. Beschrieben wird der Konnex zwischen Adipositas, Subfertilität und anovulatorischer Infertilität. Daten aus der assistierten Reproduktionstechnologie zeigen, dass die Erfolgswahrscheinlichkeit einer Fertilitätsbehandlung bei adipösen Frauen stark erniedrigt ist. Über eine Gewichtsabnahme können der Menstruationszyklus reguliert und die Wahrscheinlichkeit für eine Spontanovulation wie -konzeption erhöht werden. Dabei sollte eine moderate, andauernde Gewichtsreduktion angestrebt werden, eine radikale Gewichtsabnahme kann sich auf die Fertilität nachteilig auswirken. Eine Anleitung zu körperlicher Aktivität zusätzlich zu einer Ernährungsberatung sollte Teil des Managements bei Adipositas und Kinderwunsch sein.
Abstract
Obesity is a frequent health disorder in industrialized countries. In this review article the neuroendocrine regulation of food intake, the association of obesity with general inflammation and the occurrence of endocrine and reproductive disorders in women are discussed. It is well established that overweight women have a higher risk of ovulatory disorders and infertility. Furthermore, data from treatment with assisted reproductive techniques (ART) have shown that the likelihood of treatment success is drastically reduced in obese patients. Weight reduction can normalize menstrual cycles and increase the chance of spontaneous or assisted conception. Gradual and sustained weight loss is preferable as crash dieting can negatively impact reproductive function. Overweight patients need an integrated management including advice for physical activity and nutritional advice.
