Zusammenfassung
Das Ovarialkarzinom wird zumeist in fortgeschrittenem Stadium mit entsprechend schlechter Prognose diagnostiziert. Prävention und Screening könnten diesen fatalen Verlauf beeinflussen. Charakterisierung und Identifikation der Risikogruppen, bereits verfügbare präventive Maßnahmen fördern und mögliche in Studien evaluieren, Verbesserung der Diagnostik und Evaluation möglicher Screeningmethoden – dies sind einige Aspekte des Problemfeldes. Betrachtet werden hormonelle Kontrazeption, weitere endokrine und chemopräventive Optionen sowie chirurgische Aspekte. Die Bedeutung von nichthereditären Risiken, welche die Inzidenz des Ovarialkarzinoms erhöhen, wird erörtert. Bereits heute bestehen Interventions- und Vermeidungsmöglichkeiten zur Prävention des Ovarialkarzinoms, die mit diagnostischen Maßnahmen verbunden werden können. Sekundär- und Tertiärprävention werden diskutiert.
Abstract
Ovarian cancer is most often diagnosed at an advanced disease stage with reduced or poor prognosis. This fatal course of disease might be influenced by preventive and screening measures. Characterization and identification of populations especially at risk, promotion of already known preventive strategies, improvement of diagnostic tools, and evaluation of new compounds and potential screening instruments are some aspects for discussion. Hormonal contraception, other endocrine therapies and chemopreventive agents, and surgical interventions are critically reviewed. Factors increasing the nonhereditary risk for ovarian cancer are considered. Today, intervention and avoidance strategies are available to influence the ovarian cancer risk; these should be combined with appropriate diagnostic procedures. Secondary and tertiary prevention in ovarian cancer is discussed.
