Zusammenfassung
Laserapplikationen finden sowohl therapeutisch als auch diagnostisch Anwendung in der Gynäkologie. In vielen Kliniken hat sich die kolposkopiegesteuerte Vaporisation von Condylomata acuminata der Vulva, Vagina und Cervix uteri mittels CO2-Laser etabliert. Ebenso werden vielerorts frühe Dysplasien der entsprechenden Organe mit dem Laser behandelt. Die Konisation wird ebenfalls an einigen Kliniken mit dem Laser durchgeführt. Die hysteroskopische Durchtrennung intrauteriner Septen und Synechien kann mittels Nd:YAG-Laser oder Elektrochirurgie durchgeführt werden. Zunehmend etabliert sich in spezialisierten Zentren die fetoskopische Laserkoagulation von plazentaren Gefäßanastomosen beim fetofetalen Transfusionssyndrom. Neue laserbasierte Technologien aus dem Bereich der Nah-Infrarot-Spektroskopie erlauben nichtinvasive Messungen von Durchblutung, Wasser- und Fettgehalt im Gewebe. Diese Technologie wird zunehmend wissenschaftlich zum Studium physiologischer und pathologischer Veränderungen der Brust und der Cervix uteri eingesetzt.
Abstract
Laser-based applications are being used with increasing frequency for both diagnosis and treatment in gynaecology. Colposcopy-guided, laser-based treatment of genital warts of the vulva, vagina and uterine cervix is well established in many gynaecology departments. In some, early dysplasia is also treated by laser vaporisation. Cone biopsy is performed by laser in some departments. Hysteroscopic dissection of uterine septa or adhesions can also be performed by either laser or electrosurgery. Fetoscopy-guided laser coagulation of placental anastomoses of blood vessels in patients with feto-fetal twin-transfusion syndrome is increasingly often performed in specialised centres. New laser-based technology using near-infrared spectroscopy allows noninvasive measurement of perfusion and of water and fat content in tissues. This technology is currently used to investigate physiological and pathologic changes of the breast and the uterine cervix.
