Zusammenfassung
Im Jahr 2010 erfolgte die zweite Revision der McDonald-Kriterien für die Diagnose einer Multiplen Sklerose. Auf Basis der aktuellen Studienlage, insbesondere hinsichtlich der magnetresonanztomographischen (MRT-)Zusatzdiagnostik, konnten die diagnostischen Kriterien für den Nachweis der zeitlichen und der örtlichen Dissemination vereinfacht werden, wodurch ihre Anwendbarkeit in der Praxis erleichtert wird. Gemäß den Kriterien ist in bestimmten Fällen nun eine einzige MRT-Untersuchung für den Nachweis der örtlichen und der zeitlichen Dissemination ausreichend. Die Revision gewährleistet daher eine einfachere und raschere Diagnosestellung ohne Einschränkung von diagnostischer Sensitivität und Spezifität. Darüber hinaus lassen sich die Kriterien sowohl in Bevölkerungsgruppen unterschiedlicher geographischer Herkunft als auch bei Kindern und Jugendlichen anwenden.
Summary
In 2010, the second revision of the McDonald criteria for diagnosis of multiple sclerosis was proposed. Based on new evidence, the imaging criteria for demonstration of dissemination of central nervous system lesions in space and time have been simplified, which will allow broader application in a more uniform manner. In some circumstances, dissemination in space and time can be demonstrated in a single scan. The revision simplifies the criteria and allows accelerated diagnostic procedures while preserving their sensitivity and specificity. Additionally, the applicability across populations and in children is addressed.
