Zusammenfassung
Alkohol und Nikotin begleiten uns praktisch über die gesamte Lebensspanne. Jährlich werden rund 4000 Kinder mit einer Alkoholembryopathie und bis zu 20.000 Kinder mit der milderen Form der „fetalen Alkoholeffekte“ geboren. Bei Jugendlichen trägt Alkohol wesentlich zu Unfällen und Suizidhandlungen bei, außerdem ist der immer frühere Kontakt mit Alkohol, Nikotin und Cannabis für das noch unausgereifte jugendliche Gehirn besonders schädlich.
Im Bereich der legalen Suchtstoffe zählt Deutschland zu den Hochkonsumländern. Daraus folgend sind Suchterkrankungen bei Erwachsenen sehr häufig. Bei den über 65-Jährigen wird bis 2020 eine Verdopplung der Betroffenen erwartet mit einem massiven Anstieg alkoholbezogener Demenzen. Die derzeit verfügbaren Therapieverfahren sind erfolgreich, vor allem angesichts des chronisch rezidivierenden Charakters der Erkrankung. Allerdings werden weniger als 10% der Patienten erreicht, sodass die Angebote (auch von Psychiatern) erheblich ausgeweitet werden müssen. Während diverse Präventionsverfahren für unterschiedliche Altersgruppen erprobt und z. T. flächendeckend angeboten werden, ist die „Verhältnisprävention“, also die Regulierung des Konsums über die Preisgestaltung bzw. die Reduktion von Werbung, in Deutschland noch kaum ausgeschöpft.
Summary
Alcohol and nicotine are with us during most of our lifetime. About 4,000 children with fetal alcohol syndrome and another 20,000 children with fetal alcohol effects are born per year in Germany. Alcohol contributes to accidents and suicides especially in young people. It is particularly toxic for the developing brain.
Germany is among the countries with a high consumption of alcohol and nicotine. Consequently substance-related diseases are highly prevalent. In the group of people aged 65 and older we expect a doubling of alcohol problems within the next 10 years. This will also lead to a sharp increase in alcohol-related dementias. Overall, treatment is effective especially if one considers the chronic relapsing nature of the disorder. Unfortunately, less than 10% of patients really receive specialist care. This segment needs to be expanded especially by psychiatrists and psychotherapists. Different prevention strategies are being applied but there is a reluctance to use a ban or curtail advertising and to raise taxes for a reduction in overall consumption.
