Neuropsychologische Störungen bei amyotropher Lateralsklerose

Zusammenfassung

Die amyotrophe Lateralsklerose (ALS) wurde lange Zeit als rein motorische Systemerkrankung betrachtet. Inzwischen wissen wir, dass 2–5% der Patienten mit ALS zusätzlich an einer frontotemporalen Demenz leiden.Neuropsychologische Untersuchungen und die funktionelle Bildgebung zeigen, dass auch Patienten mit klassischer ALS neuropsychologische Defizite aufweisen können.Zu Lebzeiten nur motorisch betroffene Patienten zeigen neuropathologisch oft Atrophien des frontotemporalen Kortex.Umgekehrt finden sich bei Patienten mit frontotemporaler Demenz, die klinisch keine motorischen Einschränkungen aufweisen, neuropathologisch Atrophien der spinalen und bulbären Motoneurone.Ungeklärt ist, ob es sich bei der ALS mit neuropsychologischen Störungen um eine distinkte nosologische Entität handelt oder um einen Teil eines kontinuierlichen Spektrums, das von rein motorischen zu rein neuropsychologischen Symptomen verläuft.

Summary

Amyotrophic lateral sclerosis (ALS) has traditionally been assumed to be a purely motor disturbance. It is now recognized that 2–5% of patients with ALS develop frontotemporal dementia. Additionally, neuropsychological analysis and functional imaging suggest that a proportion of patients with classical ALS also have neuropsychological impairment. On postmortem examination, ALS patients classified as not demented showed atrophy of the frontotemporal cortex. Conversely, in patients with frontotemporal dementia without known motor impairment, atrophy of spinal and bulbar neurons were found on postmortem examination. It is still not known whether patients with ALS and neuropsychological impairment form a distinct subgroup or if they are part of a continuous spectrum that runs from pure motor impairment to pure neuropsychological impairment.
