Zusammenfassung
Komplette Rupturen des skapholunären Interkarpalbands (SL-Band) können eine dissoziative karpale Instabilität (CID) nach sich ziehen. Typisches Unfallereignis ist ein Sturz auf die Hand aus ca. 1 m Höhe oder ein Sturz beim Hand‑/Fußball. Bei entsprechender klinischer Symptomatik wie schmerzhafter Schwellung des Handgelenks zeigen nach Frakturausschluss die Röntgenzeichen einer SL-Distanz ≥3 mm, eines SL-Winkels >60° und einer Dorsalverschiebung des proximalen Skaphoidpols eine statische SL-Instabilität an. Eine komplette SL-Band-Ruptur mit dynamischer Instabilität ist in der „Clenched-fist“-Röntgenaufnahme sowie v. a. in der dynamischen Röntgendurchleuchtung (Kinematographie) gut erkennbar, die auch am Ende einer jeden distalen Radiusosteosynthese durchgeführt werden sollte. Die Therapie besteht in der sofortigen operativen Bandrefixation mit Transfixation des SL-Gelenks in perfekter Repositionsstellung innerhalb von einer bis maximal 3 Wochen. Veraltete Läsionen länger als 3 Wochen nach Unfall bedürfen der zusätzlichen Bandplastik. Hier sind einige neuere offene und arthroskopische Techniken derzeit in der Entwicklung. In postoperativen Röntgenkontrollen scheint in der seitlichen Projektion die Rezentrierung des proximalen Skaphoidpols in die Gelenkfläche des Radius einen wichtigen Parameter darzustellen.
Abstract
Complete rupture of the scapholunate (SL) ligament can cause a dissociative carpal instability (CID). These ligamentous lesions are caused by a fall from a limited height of approximately 1 m or a fall, e. g. when playing handball or soccer. For a freshly injured wrist joint, the X‑ray signs of a static instability (after excluding a fracture) are a SL distance ≥3 mm, a SL angle >60°, and a dorsal displacement of the proximal scaphoid pole. Dynamic instabilities are best seen in kinematography of the wrist joint. Early ligament refixation is mandatory, ideally done in the first week after the incident or at least within the first 3 weeks. The cornerstone of the procedure is an anatomic reduction of the SL joint stabilized with K‑wires for 8 weeks. In older lesions, ligament transfer or ligamentoplasty using a tendon transfer may add to stability but have so far not achieved a reliable joint alignment despite usually good functional results. It appears that the reduction of the proximal scaphoid pole deserves more attention.
