Zusammenfassung
Hintergrund
Die traumatische Subluxation des Radiuskopfes ist eine häufige Verletzung im Säuglings- und Kleinkindalter. Als Standardrepositionsmethode wird das Supinationsmanöver beschrieben. Die Pronationstechnik hingegen bleibt weitestgehend unerwähnt. Auf der Basis des Pronationsmanövers wurde vom Erstautor die ProFI-Reposition (Pronations-Flexions-Impression des Radiuskopfes) entwickelt.
Patienten und Methoden
Bei 41 Kindern wurde die ProFI-Methode angewandt. Als erfolgreiche Reposition wurde eine uneingeschränkte Nutzung der verletzten Extremität 10 min nach dem ProFI-Manöver definiert.
Ergebnisse
Bei den 41 Kindern war 29-mal der linke und 12-mal der rechte Arm betroffen. 17 Kinder (41 %) erlitten ein Sturzereignis. In 7 Fällen (17 %) war initial eine Fehldiagnose gestellt worden. Bei 11 Unfallverletzten (27 %) war mehr als ein Arztkontakt bis zur erfolgreichen Reposition notwendig. Die Reposition gelang mit der ProFI-Methode in allen Fällen sofort. Alle Kinder hatten nach der Einrenkung keinerlei Beschwerden mehr.
Schlussfolgerung
Die Anwendung der ProFI-Methode als modifizierte Pronationstechnik zeigte in unserem Krankengut eine exzellente Erfolgsrate. Unsere Auswertung stützt die spärliche Datenlage bezüglich der guten Repositionserfolgsrate bei Durchführung der Pronationstechniken.
Abstract
Background
Pulled elbow is a common injury in children under the age of 5 years which is usually treated by manual reduction. Supination of the forearm is recommended as opposed to pronation or other maneuvers. The author has developed a manipulative intervention for reduction of pulled elbow in young children on the basis of the pronation technique and called ProFI reduction.
Patients and methods
The ProFI method was performed on 41 children and the group was analyzed prospectively according to effectiveness of the ProFI repositioning.
Results
Among the 41 children the initial diagnosis was incorrect in 7 cases (17 %) and in 11 children (27 %) more than one doctor’s visit was necessary to reposition successfully. Repositioning with the ProFI method was immediately successful in all cases.
Conclusion
The application of the ProFI method as a modified pronation technique was shown to provide excellent effectiveness with respect to the patients treated.
