Zusammenfassung
Die Inzidenz periprothetischer Frakturen an der unteren Extremität nimmt auf Grund einer erhöhten Lebenserwartung und einer Zunahme der prothetischen Versorgung kontinuierlich zu. Das Patientengut mit periprothetischen Frakturen weist häufig bei höherem Lebensalter eine hohe Rate an Nebenerkrankungen auf. Besondere Beachtung muss die oftmals vorhandene generalisierte Osteoporose finden. Diese erfordert eine individuelle Analyse und Therapieplanung und reicht von einem konservativen Vorgehen über die Frakturstabilisierung bis zum Prothesenwechsel. Eine frühfunktionelle Behandlung sollte zur Vermeidung postoperativer Komplikationen angestrebt werden. Wo liegen die Stärken und Schwächen der zur Verfügung stehenden Versorgungsmöglichkeiten und welche indikationsspezifischen Besonderheiten sind zu erwarten? Unter Beachtung der Grundforderungen—funktionelle Weiterbehandlung ohne zusätzliche Ruhigstellung und Erhalt der Weichteilhülle durch adäquate Reposition, Osteosynthese- oder Implantatwahl—sollen die Probleme der periprothetischen Fraktur dargestellt und die zur Verfügung stehenden Versorgungsmöglichkeiten diskutiert werden.
Abstract
The incidence of periprosthetic fractures is increasing because of the increasing age and the rising number of joint replacements. Elderly patients are endangered because of a higher rate of co-morbidity such as osteoporosis or cardiovascular diseases. The treatment of periprosthetic fractures depends on these preconditions and has to solve the problem after an exact analysis of the fracture. An understanding of the biomechanical principles and risc factors is necessary for an effective treatment. Intraoperative and postoperative periprosthetic fractures will be discussed with emphasis on classification and treatment. The aim has to be an early functional postoperative treatment with partial/full weight bearing in order to avoid postoperative complications. In discussing the scope of periprosthetic fractures, the site, incidence, treatment and outcome of periprosthetic fractures of the hip and knee will be outlined.
