Epileptische Anfälle im Kindesalter Übersicht, Terminologie und Klassifikation

Zusammenfassung

Epilepsie ist eine häufige Erkrankung, epileptische Anfälle sind ein noch häufigeres Symptom. Epileptische Anfälle im Kindesalter sind vielfach schwieriger zu klassifizieren als Anfälle von Erwachsenen. Mangel an Gelegenheiten, Anfälle selbst zu sehen, ein etwas geringerer Stellenwert, den dieses Gebiet in der medizinischen Ausbildung einnimmt, und konkurrierende Vorschläge zur Anfallsklassifikation erschweren die Wahl der richtigen Terminologie. Exakte Beschreibungen und richtige Klassifizierungen epileptischer Anfälle sind jedoch unerlässliche Voraussetzungen für eine rationale Diagnostik und Therapie wie auch für eine allgemein verständliche Kommunikation von Personen und Institutionen untereinander, die Kinder mit Epilepsie betreuen bzw. behandeln. Der nachfolgende Artikel ist als kurze Übersicht über die Vielfalt epileptischer Erscheinungen im Kindesalter und als Anleitung für den nicht oder nur gelegentlich epileptologisch tätigen Kinderarzt zur Überwindung diagnostischer, terminologischer und klassifikatorischer Klippen gedacht.

Abstract

Epilepsy is one of the most common neurological diseases and epileptic seizures are even more common. Epileptic seizures in children are many times more difficult to characterize compared to seizures in adults. The lack of opportunities to personally observe seizures, the somewhat minor attention this field is paid attention to in medical training and different proposals for the classification of seizures are reason for the difficulties of physicians to choose the right terminology. The exact description and a correct classification of the epileptic events are essential requirements for rational diagnostic steps and a rational therapy. They are also necessary to enhance communication between professionals and institutions who are taking care of children with epilepsy. In this article a brief overview is given over the manifold epileptic events in children and proposals are made to pediatricians who are not specialized in epileptology on how to overcome some of the diagnostic and terminological hurdles.
