Zusammenfassung
Akute Halsschmerzen sind eine der häufigsten Ursachen für kinder- und hausärztliche Konsultationen. In den meisten Fällen liegt eine virale Ursache zugrunde, aber auch bei einer GAS-Tonsillopharyngitis (GAS: Gruppe-A-Streptokokken) ist der Spontanverlauf günstig. Um die bislang sehr häufige antibiotische Therapie bei Halsschmerzen zu begrenzen, wurde 2009 von der Deutschen Gesellschaft für Allgemeinmedizin und Familienmedizin (DEGAM), der Deutschen Gesellschaft für Kinder- und Jugendmedizin (DGKJ) und der Deutschen Gesellschaft für Pädiatrische Infektiologie (DGPI) eine Leitlinie Beratungsanlass: Halsschmerzen veröffentlicht. Nur bei ausgeprägten Symptomen und dem hochgradigen klinischen Verdacht und – bei Kindern und Jugendlichen – dem Nachweis von GAS oder aber komplizierten Verläufen wird heute eine antibiotische Therapie empfohlen. Die Indikation zur antibiotischen Therapie des klassischen Scharlachs, invasiver Infektionen oder Haut- und Weichteilinfektionen durch GAS ändert sich durch die neue Leitlinie nicht.
Abstract
Acute sore throat is one of the most common reasons for patients to visit their primary care physicians. In the majority of consultations viral pathogens seem to be causative. However, recent evidence suggests good spontaneous outcome also for group A streptococcal (GAS) tonsillopharyngitis. To specify the prescription of antibiotics for this indication the German College of General Practitioners and Family Physicians (DEGAM) together with the German Society for Pediatric and Adolescent Medicine (DGKJ) and the German Society for Pediatric Infectious Diseases (DGPI) propose a new diagnostic and therapeutic guideline: “Sore throat” in primary care. The guideline recommends the antibiotic treatment of acute sore throat only in cases of distinctive symptoms and reasonable suspicion of GAS tonsillopharyngitis as well as in cases of severe courses of disease. The indication for the antibiotic treatment does not change in view of scarlatina, invasive infections, or skin and soft tissue GAS infections.
